Common Language Description

Health and well-being coaching is a comprehensive approach that emphasizes the importance of overall wellness, extending beyond mere physical health, diet, and exercise. This coaching method aims to foster a deeper understanding of an individual's complete well-being, addressing various aspects that contribute to a person's health. The coaching process involves practical strategies designed to mitigate the impact of stress on the body, enhance nutritional knowledge, and promote increased energy and vitality. The ultimate goal is to empower individuals to become the best versions of themselves. During an initial assessment session, which is coded as CPT® 0591T, the coach may evaluate the individual's current dietary habits, nutritional requirements, sleep patterns, and personal health history, including relevant family medical history. Additionally, the assessment may consider genetic factors that could influence the individual's health, as well as the status of their relationships and current levels of stress and energy. These elements are interconnected and play a crucial role in determining the individual's overall health and wellness experience. Follow-up sessions, identified by CPT® code 0592T, are designed to last at least 30 minutes and focus on how individuals perceive their current health status in relation to their wellness goals. These sessions aim to enhance the individual's understanding of healthy behaviors and provide practical techniques to improve well-being in specific areas, such as sleep quality, vitality, exercise routines, dietary choices, social interactions, and strategies for avoiding or recovering from burnout and the physical symptoms associated with stress. For group coaching sessions involving two or more individuals, the appropriate code is CPT® 0593T, which also requires a minimum duration of 30 minutes.

The procedure for a follow-up health and well-being coaching session (CPT® 0592T) involves several key steps that ensure a comprehensive approach to the individual's wellness journey. Each session is designed to last at least 30 minutes and includes the following procedural steps:

  • Step 1: Review of Progress The session begins with a review of the individual's progress since the last meeting. This includes discussing any changes in health status, challenges faced, and successes achieved in relation to their wellness goals.
  • Step 2: Goal Assessment The coach will facilitate a discussion on how the individual perceives their current state of health in relation to their established goals. This assessment helps to identify areas that may require further attention or adjustment.
  • Step 3: Education on Healthy Behaviors The coach will provide education on healthy behaviors tailored to the individual's needs. This may include discussions on nutrition, exercise, stress management, and sleep hygiene, ensuring that the individual understands the importance of these factors in achieving overall wellness.
  • Step 4: Practical Techniques The session will introduce practical techniques aimed at enhancing well-being in specific areas. This could involve strategies for improving sleep, increasing physical activity, or managing stress effectively.
  • Step 5: Action Plan Development Together, the coach and individual will develop an action plan that outlines specific steps to be taken before the next session. This plan will be personalized to align with the individual's goals and lifestyle.

3. Post-Procedure

After the follow-up health and well-being coaching session, individuals are encouraged to implement the action plan developed during the session. This may involve making lifestyle changes, practicing new techniques for stress management, or adjusting dietary habits. Individuals should monitor their progress and be prepared to discuss their experiences in the next session. It is also important for individuals to maintain open communication with their coach regarding any challenges or successes encountered while following the action plan. Regular follow-up sessions are essential for ongoing support and to ensure that the individual remains on track toward achieving their health and wellness goals.

95 Synchronous telemedicine service rendered via a real-time interactive audio and video telecommunications system: synchronous telemedicine service is defined as a real-time interaction between a physician or other qualified health care professional and a patient who is located at a distant site from the physician or other qualified health care professional. the totality of the communication of information exchanged between the physician or other qualified health care professional and the patient during the course of the synchronous telemedicine service must be of an amount and nature that would be sufficient to meet the key components and/or requirements of the same service when rendered via a face-to-face interaction. modifier 95 may only be appended to the services listed in appendix p. appendix p is the list of cpt codes for services that are typically performed face-to-face, but may be rendered via a real-time (synchronous) interactive audio and video telecommunications system.
59 Distinct procedural service: under certain circumstances, it may be necessary to indicate that a procedure or service was distinct or independent from other non-e/m services performed on the same day. modifier 59 is used to identify procedures/services, other than e/m services, that are not normally reported together, but are appropriate under the circumstances. documentation must support a different session, different procedure or surgery, different site or organ system, separate incision/excision, separate lesion, or separate injury (or area of injury in extensive injuries) not ordinarily encountered or performed on the same day by the same individual. however, when another already established modifier is appropriate it should be used rather than modifier 59. only if no more descriptive modifier is available, and the use of modifier 59 best explains the circumstances, should modifier 59 be used. note: modifier 59 should not be appended to an e/m service. to report a separate and distinct e/m service with a non-e/m service performed on the same date, see modifier 25.
