1. Indications

The prophylactic treatment described by CPT® Code 25492 is indicated for patients with weakened bones in the radius and/or ulna due to various underlying conditions. These indications may include:

  • Bone Weakness Due to Disease The procedure is performed when the radius and/or ulna are compromised as a result of diseases that affect bone density and strength.
  • Bone Weakness Due to Neoplasm This treatment is indicated for patients with bone tumors or neoplasms that weaken the structural integrity of the radius and/or ulna, increasing the risk of fractures.

2. Procedure

The procedure for prophylactic treatment of the radius and ulna involves several key steps, which are detailed as follows:

  • Evaluation of the Weakened Bone Prior to any surgical intervention, a comprehensive evaluation of the weakened bone is conducted using radiographic imaging. This assessment helps to determine the extent of the weakness and guides the selection of the most appropriate prophylactic method.
  • Selection of Surgical Technique Based on the evaluation, the surgeon selects the most suitable technique for prophylaxis, which may include nailing, pinning, plating, or wiring. The choice is influenced by the specific condition of the bone and the degree of compromise.
  • Incision and Access An incision is made over the proximal or distal radius or ulna, depending on the chosen approach. This incision allows for direct access to the bone for the subsequent steps of the procedure.
  • Insertion of Intramedullary Nail or Rod If an intramedullary nail or rod is selected, it is inserted into the intramedullary space of the bone. This can be done using either an antegrade approach, where the nail is inserted from the end of the bone, or a retrograde approach, where it is inserted from the opposite end.
  • Securing the Nail Once the nail or rod is in place, it is secured using locking screws that are placed distally and proximally to ensure stability and prevent movement.
  • Alternative Techniques If pins are used, they may be placed transcutaneously through the weakened region of the bone. For plating, an open exposure of the bone is required, followed by the placement of a plate that is secured with screws. 3. Post-Procedure

The expected recovery time will vary based on the individual patient's condition and the complexity of the procedure performed.
