Common Language Description

The CPT® Code 28046 refers to the radical resection of a tumor, specifically a soft tissue tumor located in the foot or toe, that measures less than 3 cm. Soft tissues encompass a variety of structures, including muscles, tendons, fat, blood vessels, lymph vessels, nerves, and the tissues that surround joints. Tumors that arise in these soft tissues can be classified as either benign or malignant. For coding purposes, CPT® Code 28046 is specifically designated for the radical resection of soft tissue tumors in the foot or toe that are less than 3 cm in size, while CPT® Code 28047 is used for tumors measuring 3 cm or greater.

1. Indications

The radical resection of soft tissue tumors in the foot or toe, as described by CPT® Code 28046, is indicated for the following conditions:

  • Malignant Neoplasms - This procedure is primarily performed for malignant tumors, such as sarcomas, which require complete removal to prevent further spread of cancer.
  • Benign Tumors - In certain cases, benign tumors that may pose a risk of complications or have uncertain characteristics may also necessitate radical resection.
  • Indeterminate Tumors - Tumors that cannot be definitively classified as benign or malignant may require radical resection to ensure comprehensive evaluation and treatment.

2. Procedure

The procedure for radical resection of a soft tissue tumor in the foot or toe involves several critical steps:

  • Step 1: Incision - The surgeon begins by making a skin incision directly over the tumor site in the foot or toe. Alternatively, a skin flap may be created and elevated to provide better access to the tumor.
  • Step 2: Dissection - Once the incision is made, the surgeon carefully dissects the overlying tissue to expose the tumor. This step is crucial for visualizing the tumor and surrounding structures.
  • Step 3: Tumor Removal - The tumor is then removed en bloc, which means it is excised along with a wide margin of healthy surrounding tissue. This ensures that all potentially involved soft tissue is taken out, which may include muscles, nerves, and blood vessels.

3. Post-Procedure

Post-procedure care following a radical resection of a soft tissue tumor in the foot or toe involves monitoring for complications and ensuring proper healing. Patients may require pain management and instructions on wound care to prevent infection. The surgical site should be kept clean and dry, and any drains placed during the procedure will need to be monitored and managed appropriately. Follow-up appointments are essential to assess the healing process and to review the results of any pathology reports from the excised tissue. 58 Staged or related procedure or service by the same physician or other qualified health care professional during the postoperative period: it may be necessary to indicate that the performance of a procedure or service during the postoperative period was: (a) planned or anticipated (staged); (b) more extensive than the original procedure; or (c) for therapy following a surgical procedure. this circumstance may be reported by adding modifier 58 to the staged or related procedure. note: for treatment of a problem that requires a return to the operating/procedure room (eg, unanticipated clinical condition), see modifier 78.
59 Distinct procedural service: under certain circumstances, it may be necessary to indicate that a procedure or service was distinct or independent from other non-e/m services performed on the same day. modifier 59 is used to identify procedures/services, other than e/m services, that are not normally reported together, but are appropriate under the circumstances. documentation must support a different session, different procedure or surgery, different site or organ system, separate incision/excision, separate lesion, or separate injury (or area of injury in extensive injuries) not ordinarily encountered or performed on the same day by the same individual. however, when another already established modifier is appropriate it should be used rather than modifier 59. only if no more descriptive modifier is available, and the use of modifier 59 best explains the circumstances, should modifier 59 be used. note: modifier 59 should not be appended to an e/m service. to report a separate and distinct e/m service with a non-e/m service performed on the same date, see modifier 25.
