1. Indications

The procedure described by CPT® Code 31295 is indicated for the treatment of chronic sinusitis, particularly when there is an obstruction of the maxillary sinus ostium. This condition often leads to symptoms such as nasal congestion, facial pain or pressure, and difficulty breathing through the nose. The endoscopic dilation aims to relieve these symptoms by restoring normal drainage of the affected sinus.

  • Chronic Sinusitis A prolonged inflammation of the sinus cavities that can lead to blockage and infection.
  • Obstruction of the Maxillary Sinus Ostium A blockage at the opening of the maxillary sinus that prevents proper drainage and airflow.

2. Procedure

The procedure involves several key steps to ensure effective dilation of the maxillary sinus ostium. Initially, a topical nasal decongestant and local anesthetic with a vasoconstrictor are applied to the nasal passages to minimize discomfort and reduce bleeding during the procedure.

  • Step 1: The endoscope is introduced through the nostril and carefully advanced to the affected paranasal sinus. This allows the physician to visualize the sinus cavity and assess the extent of the obstruction.
  • Step 2: If necessary, the canine fossa may be punctured using a trocar to provide an alternative access point for dilation of the maxillary sinus. The canine fossa is located in the upper jaw, below the infraorbital opening.
  • Step 3: A guidewire catheter equipped with a small balloon is then introduced and navigated into the ostium of the blocked sinus. This step is crucial for preparing the site for dilation.
  • Step 4: The balloon is inflated to expand the ostium, which compresses the surrounding mucosa and displaces bony structures, thereby restoring the sinus drainage pathway. The inflation is carefully monitored to ensure adequate dilation.
  • Step 5: After inflation, the balloon is deflated, and the ostium is inspected with the endoscope to evaluate the effectiveness of the dilation. This inspection helps confirm that the ostium has been adequately opened.

3. Post-Procedure

After the procedure, patients may experience some discomfort, which is typically managed with prescribed pain relief medications. It is important for patients to follow any post-operative care instructions provided by their healthcare provider, which may include recommendations for nasal saline irrigation to promote healing and reduce crusting. Patients should also be monitored for any signs of complications, such as excessive bleeding or infection. Follow-up appointments may be scheduled to assess recovery and ensure that the sinus ostium remains patent, allowing for normal sinus drainage.

53 Discontinued procedure: under certain circumstances, the physician or other qualified health care professional may elect to terminate a surgical or diagnostic procedure. due to extenuating circumstances or those that threaten the well being of the patient, it may be necessary to indicate that a surgical or diagnostic procedure was started but discontinued. this circumstance may be reported by adding modifier 53 to the code reported by the individual for the discontinued procedure. note: this modifier is not used to report the elective cancellation of a procedure prior to the patient's anesthesia induction and/or surgical preparation in the operating suite. for outpatient hospital/ambulatory surgery center (asc) reporting of a previously scheduled procedure/service that is partially reduced or cancelled as a result of extenuating circumstances or those that threaten the well being of the patient prior to or after administration of anesthesia, see modifiers 73 and 74 (see modifiers approved for asc hospital outpatient use).
