The procedure described by CPT® Code 47000 refers to a percutaneous biopsy of the liver, which is a minimally invasive technique used to obtain a tissue sample from the liver for diagnostic purposes. In this procedure, the skin over the liver is first cleansed to reduce the risk of infection, and a local anesthetic is administered to ensure patient comfort during the biopsy. The term "percutaneous" indicates that the biopsy is performed through the skin, utilizing a needle to access the liver directly. This method allows for the collection of liver tissue without the need for open surgery, making it a preferred choice in many clinical situations.
During the biopsy, imaging guidance may be employed to enhance the accuracy of the procedure. Techniques such as ultrasound, fluoroscopy, computed tomography (CT), or magnetic resonance imaging (MRI) can be used to visualize the liver and identify the specific area of abnormal tissue that needs to be sampled. The biopsy is performed using a large-bore needle or a spring-loaded biopsy gun, which is inserted through the skin and into the liver to withdraw a tissue sample. This sample is then sent to a laboratory for analysis, where it can be examined for various conditions, including liver disease, tumors, or infections.
It is important to note that if a needle biopsy of the liver is performed during another major surgical procedure, the appropriate code to use is 47001, which accounts for the biopsy being conducted through an existing abdominal incision rather than as a standalone procedure.

The percutaneous liver biopsy, as described by CPT® Code 47000, is indicated for several clinical scenarios where liver tissue analysis is necessary. The following conditions may warrant this procedure:
The procedure for a percutaneous liver biopsy involves several critical steps to ensure safety and accuracy. First, the patient is positioned appropriately, typically lying on their back, to allow easy access to the liver. The skin over the liver is then thoroughly cleansed with an antiseptic solution to minimize the risk of infection. Following this, a local anesthetic is injected into the skin and underlying tissues to numb the area where the biopsy will be performed, ensuring that the patient experiences minimal discomfort during the procedure.
Once the area is anesthetized, imaging guidance may be employed to accurately locate the area of interest within the liver. This guidance can be provided by ultrasound, fluoroscopy, CT, or MRI, depending on the clinical situation and available resources. The imaging helps the physician visualize the liver and identify any abnormal tissue that requires sampling.
After the target area is confirmed, a large-bore needle or a spring-loaded biopsy gun is carefully inserted through the skin and into the liver. The physician will then withdraw a tissue sample from the liver using the needle. This sample is typically a small cylinder of liver tissue, which is then placed in a specimen container for further analysis in a laboratory. The entire procedure is usually completed within a short time frame, often taking less than 30 minutes.
After the percutaneous liver biopsy, the patient is typically monitored for a short period to ensure there are no immediate complications, such as bleeding or infection. It is common for patients to experience some discomfort or soreness at the biopsy site, which can usually be managed with over-the-counter pain relief medications. Patients are often advised to avoid strenuous activities for a short period following the procedure to allow for proper healing. Additionally, follow-up appointments may be scheduled to discuss the biopsy results and any necessary further management based on the findings.
