Common Language Description

Paravertebral facet joints, also known as zygapophyseal joints, are critical structures located at the posterior aspect of the spine, situated on either side of the vertebrae where one vertebra overlaps another. These joints play a significant role in spinal movement and stability. Pain in the facet joints can arise from various conditions, including post-laminectomy syndrome, which may occur after spinal surgery due to destabilization of the spinal joints, the formation of scar tissue, or recurrent disc herniation. Other potential causes of facet joint pain include degenerative conditions such as spondylosis, spondylolisthesis, and arthritis. To address this pain, a procedure involving the injection of a diagnostic or therapeutic agent into the facet joint or the nerves that innervate the joint is performed. This procedure is conducted with the aid of imaging guidance, either fluoroscopy or computed tomography (CT), to ensure accurate placement of the injection. Initially, the skin over the facet joint is sterilized, and a local anesthetic is administered to minimize discomfort. A spinal needle is then carefully directed into the facet joint space until it contacts bone or cartilage. To confirm the correct positioning of the needle, a small amount of contrast material is injected. Following this verification, a local anesthetic and/or steroid is injected into the joint. The diagnostic facet joint injection primarily utilizes a local anesthetic to pinpoint the specific source of pain. If the patient experiences significant pain relief after this diagnostic injection, a therapeutic injection may be scheduled for a later date, which typically involves a long-acting local anesthetic combined with a steroid. For coding purposes, CPT® Code 64490 is used for a single cervical or thoracic facet joint injection, 64491 for the second level, and 64492 for the third and any additional cervical or thoracic levels injected.

1. Indications

Facet joint injections are indicated for various conditions that may cause pain in the cervical or thoracic regions of the spine. The following are explicitly provided indications for this procedure:

  • Post-Laminectomy Syndrome - Pain that persists following spinal surgery, often due to destabilization of the spinal joints or scar tissue formation.
  • Recurrent Disc Herniation - Pain resulting from the reoccurrence of a herniated disc that may affect the facet joints.
  • Spondylosis - Degenerative changes in the spine that can lead to pain and discomfort in the facet joints.
  • Spondylolisthesis - A condition where one vertebra slips over another, potentially causing facet joint pain.
  • Arthritis - Inflammation of the facet joints due to arthritic conditions can lead to significant pain and discomfort.

2. Procedure

The procedure for performing a facet joint injection involves several critical steps to ensure accuracy and patient safety. The following procedural steps are outlined:

  • Step 1: Preparation - The skin over the targeted facet joint is thoroughly cleaned and sterilized to minimize the risk of infection. A local anesthetic is then injected to numb the area, ensuring the patient experiences minimal discomfort during the procedure.
  • Step 2: Needle Insertion - A spinal needle is carefully inserted into the facet joint space. The physician directs the needle until it makes contact with bone or cartilage, which indicates that the needle is in the correct position.
  • Step 3: Contrast Injection - A small amount of contrast material is injected through the needle to confirm its proper placement within the facet joint. This step is crucial for ensuring that the injection is delivered accurately to the intended site.
  • Step 4: Therapeutic Injection - Following the confirmation of needle placement, a local anesthetic and/or steroid is injected into the facet joint. This injection aims to provide pain relief and reduce inflammation in the joint.
  • Step 5: Diagnostic Assessment - If the injection is performed for diagnostic purposes, the local anesthetic helps identify the specific area generating pain. If significant pain relief is experienced by the patient, a therapeutic injection may be scheduled for a later date.

3. Post-Procedure

After the facet joint injection, patients are typically monitored for a short period to assess for any immediate adverse reactions. It is common for patients to experience some soreness at the injection site, which may resolve within a few days. Patients are advised to avoid strenuous activities for a short period following the procedure to allow for proper healing. Depending on the response to the injection, follow-up appointments may be scheduled to evaluate the effectiveness of the treatment and to determine if further therapeutic injections are necessary. Additionally, patients should be informed about potential side effects, such as temporary increased pain or swelling, and advised to contact their healthcare provider if they experience any concerning symptoms.

53 Discontinued procedure: under certain circumstances, the physician or other qualified health care professional may elect to terminate a surgical or diagnostic procedure. due to extenuating circumstances or those that threaten the well being of the patient, it may be necessary to indicate that a surgical or diagnostic procedure was started but discontinued. this circumstance may be reported by adding modifier 53 to the code reported by the individual for the discontinued procedure. note: this modifier is not used to report the elective cancellation of a procedure prior to the patient's anesthesia induction and/or surgical preparation in the operating suite. for outpatient hospital/ambulatory surgery center (asc) reporting of a previously scheduled procedure/service that is partially reduced or cancelled as a result of extenuating circumstances or those that threaten the well being of the patient prior to or after administration of anesthesia, see modifiers 73 and 74 (see modifiers approved for asc hospital outpatient use).

74 Discontinued out-patient hospital/ambulatory surgery center (asc) procedure after administration of anesthesia: due to extenuating circumstances or those that threaten the well being of the patient, the physician may terminate a surgical or diagnostic procedure after the administration of anesthesia (local, regional block(s), general) or after the procedure was started (incision made, intubation started, scope inserted, etc). under these circumstances, the procedure started but terminated can be reported by its usual procedure number and the addition of modifier 74. note: the elective cancellation of a service prior to the administration of anesthesia and/or surgical preparation of the patient should not be reported. for physician reporting of a discontinued procedure, see modifier 53.
