Common Language Description

Paravertebral facet joints, also known as zygapophyseal joints, are critical structures located on the posterior aspect of the spine, situated on either side of the vertebrae where one vertebra overlaps another. These joints play a significant role in spinal movement and stability. Pain in the facet joints can arise from various conditions, including post-laminectomy syndrome, which may occur after spinal surgery due to destabilization of the spinal joints, formation of scar tissue, or recurrent disc herniation. Other contributing factors to facet joint pain include degenerative conditions such as spondylosis, spondylolisthesis, and arthritis. To address this pain, a procedure involving the injection of a diagnostic or therapeutic agent into the facet joint or the nerves that innervate the joint is performed. This procedure is conducted with the aid of imaging guidance, either fluoroscopy or computed tomography (CT), to ensure accurate placement of the injection. Initially, the skin over the facet joint is sterilized, and a local anesthetic is administered to minimize discomfort. A spinal needle is then carefully inserted into the facet joint space until it contacts bone or cartilage. To confirm the correct positioning of the needle, a small amount of contrast material is injected. Following this verification, a local anesthetic and/or steroid is injected into the joint. The diagnostic facet joint injection primarily utilizes a local anesthetic to pinpoint the specific source of pain. If the patient experiences significant pain relief after this diagnostic injection, a therapeutic injection may be scheduled for a later date, which typically involves a long-acting local anesthetic combined with a steroid. For coding purposes, a single lumbar or sacral facet joint injection is represented by CPT® Code 64493, the second level by CPT® Code 64494, and the third and any additional levels by CPT® Code 64495.

1. Indications

The procedure involving the injection of a diagnostic or therapeutic agent into the paravertebral facet joints is indicated for several conditions that may cause pain and discomfort in the lumbar or sacral regions. These indications include:

  • Post-Laminectomy Syndrome - Pain that persists following spinal surgery, often due to destabilization of the spinal joints or scar tissue formation.
  • Recurrent Disc Herniation - The reoccurrence of a herniated disc that can lead to pain and discomfort in the facet joints.
  • Spondylosis - A degenerative condition affecting the spine, which can lead to pain in the facet joints.
  • Spondylolisthesis - A condition where one vertebra slips over another, potentially causing pain in the facet joints.
  • Arthritis - Inflammation of the joints, including the facet joints, which can lead to pain and reduced mobility.

2. Procedure

The procedure for injecting a diagnostic or therapeutic agent into the paravertebral facet joints involves several key steps, each critical for ensuring accuracy and patient safety. The steps are as follows:

  • Preparation of the Skin - The area over the facet joint is thoroughly cleaned and sterilized to minimize the risk of infection before the injection is performed.
  • Administration of Local Anesthetic - A local anesthetic is injected into the skin over the facet joint to reduce discomfort during the procedure.
  • Insertion of the Spinal Needle - A spinal needle is carefully directed into the facet joint space. The physician advances the needle until it makes contact with bone or cartilage, ensuring that it is correctly positioned within the joint.
  • Verification of Needle Position - A small amount of contrast material is injected through the needle to confirm that it is accurately placed within the facet joint space. This step is crucial for ensuring that the subsequent injection is delivered to the correct location.
  • Injection of Diagnostic or Therapeutic Agent - Once the needle position is verified, a local anesthetic and/or steroid is injected into the facet joint. This injection may serve a diagnostic purpose, helping to identify the source of pain, or a therapeutic purpose, providing relief from pain and inflammation.

3. Post-Procedure

After the injection procedure, patients are typically monitored for a short period to assess for any immediate adverse reactions. It is common for patients to experience some soreness at the injection site, which may resolve within a few days. Patients may be advised to avoid strenuous activities for a brief period following the injection to allow for optimal recovery. If the diagnostic injection provides significant pain relief, a follow-up appointment may be scheduled for a therapeutic injection using a long-acting local anesthetic and steroid. It is important for patients to report any unusual symptoms or prolonged pain following the procedure to their healthcare provider for further evaluation.

53 Discontinued procedure: under certain circumstances, the physician or other qualified health care professional may elect to terminate a surgical or diagnostic procedure. due to extenuating circumstances or those that threaten the well being of the patient, it may be necessary to indicate that a surgical or diagnostic procedure was started but discontinued. this circumstance may be reported by adding modifier 53 to the code reported by the individual for the discontinued procedure. note: this modifier is not used to report the elective cancellation of a procedure prior to the patient's anesthesia induction and/or surgical preparation in the operating suite. for outpatient hospital/ambulatory surgery center (asc) reporting of a previously scheduled procedure/service that is partially reduced or cancelled as a result of extenuating circumstances or those that threaten the well being of the patient prior to or after administration of anesthesia, see modifiers 73 and 74 (see modifiers approved for asc hospital outpatient use).
56 Preoperative management only: when 1 physician or other qualified health care professional performed the preoperative care and evaluation and another performed the surgical procedure, the preoperative component may be identified by adding modifier 56 to the usual procedure number.
58 Staged or related procedure or service by the same physician or other qualified health care professional during the postoperative period: it may be necessary to indicate that the performance of a procedure or service during the postoperative period was: (a) planned or anticipated (staged); (b) more extensive than the original procedure; or (c) for therapy following a surgical procedure. this circumstance may be reported by adding modifier 58 to the staged or related procedure. note: for treatment of a problem that requires a return to the operating/procedure room (eg, unanticipated clinical condition), see modifier 78.
73 Discontinued out-patient hospital/ambulatory surgery center (asc) procedure prior to the administration of anesthesia: due to extenuating circumstances or those that threaten the well being of the patient, the physician may cancel a surgical or diagnostic procedure subsequent to the patient's surgical preparation (including sedation when provided, and being taken to the room where the procedure is to be performed), but prior to the administration of anesthesia (local, regional block(s) or general). under these circumstances, the intended service that is prepared for but cancelled can be reported by its usual procedure number and the addition of modifier 73. note: the elective cancellation of a service prior to the administration of anesthesia and/or surgical preparation of the patient should not be reported. for physician reporting of a discontinued procedure, see modifier 53.
74 Discontinued out-patient hospital/ambulatory surgery center (asc) procedure after administration of anesthesia: due to extenuating circumstances or those that threaten the well being of the patient, the physician may terminate a surgical or diagnostic procedure after the administration of anesthesia (local, regional block(s), general) or after the procedure was started (incision made, intubation started, scope inserted, etc). under these circumstances, the procedure started but terminated can be reported by its usual procedure number and the addition of modifier 74. note: the elective cancellation of a service prior to the administration of anesthesia and/or surgical preparation of the patient should not be reported. for physician reporting of a discontinued procedure, see modifier 53.
78 Unplanned return to the operating/procedure room by the same physician or other qualified health care professional following initial procedure for a related procedure during the postoperative period: it may be necessary to indicate that another procedure was performed during the postoperative period of the initial procedure (unplanned procedure following initial procedure). when this procedure is related to the first, and requires the use of an operating/procedure room, it may be reported by adding modifier 78 to the related procedure. (for repeat procedures, see modifier 76.)
80 Assistant surgeon: surgical assistant services may be identified by adding modifier 80 to the usual procedure number(s).
81 Minimum assistant surgeon: minimum surgical assistant services are identified by adding modifier 81 to the usual procedure number.
82 Assistant surgeon (when qualified resident surgeon not available): the unavailability of a qualified resident surgeon is a prerequisite for use of modifier 82 appended to the usual procedure code number(s).
95 Synchronous telemedicine service rendered via a real-time interactive audio and video telecommunications system: synchronous telemedicine service is defined as a real-time interaction between a physician or other qualified health care professional and a patient who is located at a distant site from the physician or other qualified health care professional. the totality of the communication of information exchanged between the physician or other qualified health care professional and the patient during the course of the synchronous telemedicine service must be of an amount and nature that would be sufficient to meet the key components and/or requirements of the same service when rendered via a face-to-face interaction. modifier 95 may only be appended to the services listed in appendix p. appendix p is the list of cpt codes for services that are typically performed face-to-face, but may be rendered via a real-time (synchronous) interactive audio and video telecommunications system.
