Common Language Description

The procedure described by CPT® Code 68841 involves the insertion of a drug-eluting implant into the lacrimal canaliculus, which is a small duct that drains tears from the eye. This procedure may also include punctal dilation if necessary. The lacrimal canaliculus connects to the lacrimal puncta, located at the inner corners of each eye, where tears are collected from the lacrimal lake. The drug-eluting implant, often referred to as an intracanalicular plug, is made from an absorbable polyethylene glycol hydrogel that is infused with medication, such as dexamethasone or travoprost. Dexamethasone is typically used to manage postoperative inflammation and pain, while travoprost is indicated for the treatment of glaucoma and ocular hypertension. The implant is designed to provide a sustained release of the medication directly into the eye, allowing for effective treatment over time. As the medication is absorbed, it is gradually released into the ocular surface and subsequently exits through the nasolacrimal system. 1. Indications

The procedure associated with CPT® Code 68841 is indicated for specific conditions that require targeted treatment through the lacrimal system. The following are the primary indications for this procedure:

  • Postoperative Inflammation and Pain The insertion of the drug-eluting implant containing dexamethasone is indicated for managing inflammation and pain following ocular surgeries.
  • Glaucoma The use of the implant containing travoprost is indicated for the treatment of glaucoma, a condition characterized by increased intraocular pressure that can lead to optic nerve damage.
  • Ocular Hypertension The procedure is also indicated for patients with ocular hypertension, where the pressure inside the eye is elevated, potentially leading to glaucoma.

2. Procedure

The procedure for the insertion of the drug-eluting implant into the lacrimal canaliculus involves several key steps, which are detailed below:

  • Step 1: Preparation The patient is positioned comfortably, and the eye area is prepared for the procedure. This may involve cleaning the surrounding skin and administering topical anesthetic drops to minimize discomfort during the insertion.
  • Step 2: Punctal Dilation (if performed) If necessary, the lacrimal puncta may be dilated to facilitate the insertion of the implant. This is done using a specialized instrument to gently widen the opening of the puncta, allowing for easier access to the canaliculus.

3. Post-Procedure

53 Discontinued procedure: under certain circumstances, the physician or other qualified health care professional may elect to terminate a surgical or diagnostic procedure. due to extenuating circumstances or those that threaten the well being of the patient, it may be necessary to indicate that a surgical or diagnostic procedure was started but discontinued. this circumstance may be reported by adding modifier 53 to the code reported by the individual for the discontinued procedure. note: this modifier is not used to report the elective cancellation of a procedure prior to the patient's anesthesia induction and/or surgical preparation in the operating suite. for outpatient hospital/ambulatory surgery center (asc) reporting of a previously scheduled procedure/service that is partially reduced or cancelled as a result of extenuating circumstances or those that threaten the well being of the patient prior to or after administration of anesthesia, see modifiers 73 and 74 (see modifiers approved for asc hospital outpatient use).
54 Surgical care only: when 1 physician or other qualified health care professional performs a surgical procedure and another provides preoperative and/or postoperative management, surgical services may be identified by adding modifier 54 to the usual procedure number.

78 Unplanned return to the operating/procedure room by the same physician or other qualified health care professional following initial procedure for a related procedure during the postoperative period: it may be necessary to indicate that another procedure was performed during the postoperative period of the initial procedure (unplanned procedure following initial procedure). when this procedure is related to the first, and requires the use of an operating/procedure room, it may be reported by adding modifier 78 to the related procedure. (for repeat procedures, see modifier 76.)
82 Assistant surgeon (when qualified resident surgeon not available): the unavailability of a qualified resident surgeon is a prerequisite for use of modifier 82 appended to the usual procedure code number(s).
97 Rehabilitative services: when a service or procedure that may be either habilitative or rehabilitative in nature is provided for rehabilitative purposes, the physician or other qualified health care professional may add modifier 97 to the service or procedure code to indicate that the service or procedure provided was a rehabilitative service. rehabilitative services help an individual keep, get back, or improve skills and functioning for daily living that have been lost or impaired because the individual was sick, hurt, or disabled.
