A diagnostic radiologic examination, specifically CPT® Code 72081, involves a comprehensive assessment of the entire thoracic and lumbar spine, which may also include the skull, cervical, and sacral spine if performed. This examination is particularly utilized for scoliosis evaluation, allowing healthcare professionals to determine the type of scoliosis present, as well as the location and degree of curvature in the spine. The procedure employs X-ray technology, which utilizes indirect ionizing radiation to create images of the internal structures of the body. X-rays are effective in imaging non-uniform materials, such as human tissue, due to the varying densities and compositions of these materials. This differential absorption of X-rays results in some rays being absorbed while others pass through, ultimately being captured on a detector to produce a two-dimensional (2D) image of the spinal structures. Code 72081 specifically denotes a single view X-ray of the thoracic and lumbar spine, distinguishing it from other codes that represent multiple views. For instance, CPT® Code 72082 indicates 2 or 3 views, while Code 72083 covers 4 or 5 views, and Code 72084 is used for evaluations that require a minimum of 6 views. Typically, posteroanterior, frontal, and lateral views are obtained while the patient is in an erect, standing, or upright position, which aids in the assessment of lateral curvature. During the procedure, the patient is positioned in front of a vertical grid with their knees together and fully extended, ensuring that the entire thoracic and lumbar spine, along with the cervical and sacral areas and the skull, are included in the field of view. The examination also involves measuring the vertebral bodies above and below the apex of the spinal curve, which are the most tilted, by using intersecting lines to determine the degree of curvature. Additionally, lateral projections may be captured with the patient's arms extended straight out in front to enhance the visibility of the curvature, and other views may be taken while the patient is lying supine.

1. Indications

The radiologic examination represented by CPT® Code 72081 is indicated for the evaluation of scoliosis. This procedure is performed to assess various aspects of scoliosis, including the type of scoliosis present, the location of the curvature, and the degree of curvature in the thoracic and lumbar spine. It is particularly useful in cases where a comprehensive view of the spine is necessary to determine the extent of the condition and to guide further management or treatment.

  • Scoliosis Evaluation This examination is specifically indicated for assessing scoliosis, allowing for the identification of the type and severity of the spinal curvature.

2. Procedure

The procedure for CPT® Code 72081 involves several key steps to ensure a comprehensive radiologic examination of the thoracic and lumbar spine.

  • Patient Positioning The patient is positioned in front of a vertical grid, standing upright with their knees together and fully extended. This positioning is crucial for obtaining accurate images of the spine.
  • Field of View The field of view is carefully set to include the entire thoracic and lumbar spine, as well as the cervical and sacral spinal areas and the skull, if necessary. This ensures that all relevant structures are captured in the imaging process.
  • Image Acquisition A single view X-ray is taken, typically in a posteroanterior or lateral projection. The X-ray machine emits ionizing radiation, which passes through the body and is captured on a detector, producing a two-dimensional image of the spinal structures.
  • Curvature Measurement After the images are obtained, the vertebral bodies above and below the apex of the spinal curve, which are the most tilted, are measured using intersecting lines. This measurement provides the degree of curvature, which is essential for evaluating the severity of scoliosis.
  • Additional Views If necessary, lateral projections may be taken with the patient’s arms extended straight out in front to enhance the visibility of the curvature. Other views may also be captured while the patient is lying supine to provide a more comprehensive assessment of the spine.

3. Post-Procedure

Post-procedure care for a radiologic examination using CPT® Code 72081 typically involves minimal recovery time, as the procedure is non-invasive and does not require anesthesia. Patients may resume normal activities immediately following the examination. However, it is essential for healthcare providers to review the obtained images and measurements to determine the next steps in management or treatment based on the findings. Any necessary follow-up appointments or additional imaging studies may be scheduled based on the results of the examination.

25 Significant, separately identifiable evaluation and management service by the same physician or other qualified health care professional on the same day of the procedure or other service: it may be necessary to indicate that on the day a procedure or service identified by a cpt code was performed, the patient's condition required a significant, separately identifiable e/m service above and beyond the other service provided or beyond the usual preoperative and postoperative care associated with the procedure that was performed. a significant, separately identifiable e/m service is defined or substantiated by documentation that satisfies the relevant criteria for the respective e/m service to be reported (see evaluation and management services guidelines for instructions on determining level of e/m service). the e/m service may be prompted by the symptom or condition for which the procedure and/or service was provided. as such, different diagnoses are not required for reporting of the e/m services on the same date. this circumstance may be reported by adding modifier 25 to the appropriate level of e/m service. note: this modifier is not used to report an e/m service that resulted in a decision to perform surgery. see modifier 57 for significant, separately identifiable non-e/m services, see modifier 59.
51 Multiple procedures: when multiple procedures, other than e/m services, physical medicine and rehabilitation services or provision of supplies (eg, vaccines), are performed at the same session by the same individual, the primary procedure or service may be reported as listed. the additional procedure(s) or service(s) may be identified by appending modifier 51 to the additional procedure or service code(s). note: this modifier should not be appended to designated "add-on" codes (see appendix d).
59 Distinct procedural service: under certain circumstances, it may be necessary to indicate that a procedure or service was distinct or independent from other non-e/m services performed on the same day. modifier 59 is used to identify procedures/services, other than e/m services, that are not normally reported together, but are appropriate under the circumstances. documentation must support a different session, different procedure or surgery, different site or organ system, separate incision/excision, separate lesion, or separate injury (or area of injury in extensive injuries) not ordinarily encountered or performed on the same day by the same individual. however, when another already established modifier is appropriate it should be used rather than modifier 59. only if no more descriptive modifier is available, and the use of modifier 59 best explains the circumstances, should modifier 59 be used. note: modifier 59 should not be appended to an e/m service. to report a separate and distinct e/m service with a non-e/m service performed on the same date, see modifier 25.
76 Repeat procedure or service by same physician or other qualified health care professional: it may be necessary to indicate that a procedure or service was repeated by the same physician or other qualified health care professional subsequent to the original procedure or service. this circumstance may be reported by adding modifier 76 to the repeated procedure or service. note: this modifier should not be appended to an e/m service.
