Official Description

Ultrasound, abdominal aorta, real time with image documentation, screening study for abdominal aortic aneurysm (AAA)

Common Language Description

The CPT® Code 76706 refers to a specific ultrasound procedure known as a real-time screening study for abdominal aortic aneurysm (AAA). This procedure utilizes ultrasound technology to assess the abdominal aorta, which is the major artery responsible for delivering oxygenated blood from the heart to the rest of the body. The aorta is divided into two main sections: the upper portion located in the chest and the lower portion situated deep within the abdomen, adjacent to the spine. Over time, the walls of the aorta can become weakened due to various degenerative conditions, particularly atherosclerosis, a disease characterized by the buildup of plaque in the arterial walls. This weakening can lead to the formation of an aneurysm, where the affected area of the aorta bulges outward, resembling a balloon. Abdominal aortic aneurysms are often asymptomatic until they reach a critical stage, such as rupture, which can result in severe internal bleeding and is potentially life-threatening. Certain populations are identified as being at higher risk for developing an AAA, including men aged 65 to 75 who have a history of tobacco use, as well as individuals with a family history of AAA or a personal history of hypertension. For these at-risk groups, a one-time ultrasound screening is recommended to detect any abnormalities in the aorta before they progress to more serious conditions. During the procedure, the patient is positioned supine on an examination table, and a conductive gel is applied to the abdominal skin to facilitate the transmission of ultrasound waves. A technician then maneuvers the ultrasound transducer across the abdomen, capturing real-time images of the aorta and the blood flow within it. These images are displayed on a computer monitor and can be saved in both still and real-time formats for subsequent review and interpretation by a radiologist.

1. Indications

The ultrasound procedure coded as CPT® 76706 is indicated for screening at-risk individuals for abdominal aortic aneurysm (AAA). The following conditions and risk factors warrant this screening:

  • Age and Gender: Men aged 65 to 75 years are recommended for screening due to their higher risk of developing AAA.
  • Tobacco Use: Individuals with a positive history of tobacco use are at an increased risk for AAA and should undergo screening.
  • Family History: Patients with a family history of abdominal aortic aneurysm are advised to have this screening to monitor for potential aneurysm development.
  • Personal History of Hypertension: Those with a personal history of hypertension are also considered at greater risk and should be screened for AAA.

2. Procedure

The procedure for CPT® 76706 involves several key steps to ensure accurate imaging of the abdominal aorta:

  • Patient Positioning: The patient is instructed to lie supine on the examination table, which allows optimal access to the abdominal area for the ultrasound examination.
  • Application of Gel: An ultrasonic conduction gel is applied to the skin of the abdomen. This gel is essential as it eliminates air pockets between the skin and the ultrasound transducer, facilitating the transmission of sound waves.
  • Ultrasound Imaging: A trained technician uses an ultrasound transducer, which emits sound waves, to scan the abdominal area. The technician moves the transducer across various regions of the abdomen to capture comprehensive images of the abdominal aorta and assess blood flow within the vessel.
  • Image Documentation: The images obtained during the procedure are displayed on a computer monitor in real-time. The technician can save these images in still and/or real-time formats for further analysis.
  • Interpretation: The saved images are later reviewed and interpreted by a radiologist, who will assess the presence of any abnormalities, such as an aneurysm.

3. Post-Procedure

After the completion of the ultrasound procedure, there are typically no specific post-procedure care requirements for the patient. The patient can resume normal activities immediately following the examination. The results of the ultrasound will be compiled and interpreted by a radiologist, who will provide a report detailing any findings related to the abdominal aorta. This report is then communicated to the referring physician, who will discuss the results with the patient and determine if any further action or follow-up is necessary based on the findings.

This is a primary code that can be used with these additional add-on codes.

G0513 Telehealth Service (Medicare) Medicare Coverage: Carrier Priced Add-on Code MPFS Status: Active Code APC N Prolonged preventive service(s) (beyond the typical service time of the primary procedure), in the office or other outpatient setting requiring direct patient contact beyond the usual service; first 30 minutes (list separately in addition to code for preventive service)
26 Professional component: certain procedures are a combination of a physician or other qualified health care professional component and a technical component. when the physician or other qualified health care professional component is reported separately, the service may be identified by adding modifier 26 to the usual procedure number.
X5 Diagnostic services requested by another clinician: for reporting services by a clinician who furnishes care to the patient only as requested by another clinician or subsequent and related services requested by another clinician; this modifier is reported for patient relationships that may not be adequately captured by the above alternative categories; reporting clinician service examples include but are not limited to, the radiologist's interpretation of an imaging study requested by another clinician
TC Technical component; under certain circumstances, a charge may be made for the technical component alone; under those circumstances the technical component charge is identified by adding modifier 'tc' to the usual procedure number; technical component charges are institutional charges and not billed separately by physicians; however, portable x-ray suppliers only bill for technical component and should utilize modifier tc; the charge data from portable x-ray suppliers will then be used to build customary and prevailing profiles
GC This service has been performed in part by a resident under the direction of a teaching physician
GA Waiver of liability statement issued as required by payer policy, individual case
59 Distinct procedural service: under certain circumstances, it may be necessary to indicate that a procedure or service was distinct or independent from other non-e/m services performed on the same day. modifier 59 is used to identify procedures/services, other than e/m services, that are not normally reported together, but are appropriate under the circumstances. documentation must support a different session, different procedure or surgery, different site or organ system, separate incision/excision, separate lesion, or separate injury (or area of injury in extensive injuries) not ordinarily encountered or performed on the same day by the same individual. however, when another already established modifier is appropriate it should be used rather than modifier 59. only if no more descriptive modifier is available, and the use of modifier 59 best explains the circumstances, should modifier 59 be used. note: modifier 59 should not be appended to an e/m service. to report a separate and distinct e/m service with a non-e/m service performed on the same date, see modifier 25.
Q6 Service furnished under a fee-for-time compensation arrangement by a substitute physician or by a substitute physical therapist furnishing outpatient physical therapy services in a health professional shortage area, a medically underserved area, or a rural area
