Common Language Description

The CPT® Code 76981 refers to a specialized ultrasound procedure known as elastography, which is utilized to assess the elastic properties of soft tissues, particularly the parenchyma of organs or lesions located within these soft tissues. Parenchymal tissue constitutes the primary functional cells of an organ, making this imaging technique vital for diagnosing various conditions affecting organs such as the liver, breast, thyroid, prostate, and muscles. Elastography employs different ultrasound techniques to generate both qualitative and quantitative images that map the elasticity of the target tissue. One common method, strain elastography, utilizes real-time two-dimensional visualization to create an elastographic map by applying low-frequency compression through a handheld ultrasound transducer or by leveraging physiological movements, such as breathing or pulsation, to induce tissue displacement. This technique is predominantly used in musculoskeletal imaging. Another method, acoustic radiation force impulse (ARFI) elastography, involves the internal excitation of tissue via a focused ultrasound pulse, resulting in a qualitative color-coded or gray-scale representation of tissue stiffness. ARFI is particularly effective for deep-tissue imaging, including assessments of the liver, thyroid, and breast. Additionally, shear wave elasticity imaging (SWEI) generates images by measuring the sideways propagation of ultrasound waves produced by the transducer, making it suitable for deep-tissue evaluations. A variant of SWEI, known as transient vibration controlled elastography, employs short bursts of vibration specifically for imaging the liver. The CPT® Code 76981 is designated for the evaluation of a parenchymal organ and any specific lesions within that organ, while codes 76982 and 76983 are used for the evaluation of specific soft tissue lesions and additional target lesions, respectively.

1. Indications

The procedure associated with CPT® Code 76981 is indicated for various conditions and diagnostic purposes related to soft tissue organs. The following are the explicitly provided indications for performing this ultrasound elastography:

  • Assessment of Liver Conditions This procedure is utilized to evaluate liver diseases, including fibrosis and cirrhosis, by measuring the stiffness of liver tissue.
  • Breast Lesion Evaluation Elastography can be employed to assess the elasticity of breast tissue, aiding in the diagnosis of benign and malignant lesions.
  • Thyroid Disorders The technique is indicated for examining thyroid tissue to identify abnormalities such as nodules or other thyroid-related conditions.

2. Procedure

The procedure for CPT® Code 76981 involves several key steps to ensure accurate assessment of the elastic properties of the target tissue. The following procedural steps are outlined:

  • Preparation of the Patient The patient is positioned comfortably, and the area of interest is exposed. A coupling gel is applied to the skin to facilitate the transmission of ultrasound waves.
  • Application of Ultrasound Transducer A handheld ultrasound transducer is placed on the skin over the targeted organ or lesion. The operator may apply gentle pressure to ensure optimal contact and enhance image quality.
  • Induction of Tissue Displacement The operator may utilize low-frequency compression techniques or rely on physiological movements, such as breathing, to induce tissue displacement. This step is crucial for generating the elastographic map.
  • Image Acquisition The ultrasound machine captures real-time images of the tissue, producing an elastographic map that displays the elastic properties of the parenchyma or lesion. The images may be qualitative (color-coded) or quantitative (gray-scale) based on the elastography technique used.
  • Analysis of Results The acquired images are analyzed to assess the stiffness of the tissue, which can provide diagnostic information regarding potential diseases or abnormalities present in the organ or lesion.

3. Post-Procedure

After the completion of the elastography procedure, the patient may be monitored briefly to ensure there are no immediate adverse effects. Typically, there are no specific post-procedure care requirements, and patients can resume normal activities immediately. The results of the elastography will be documented and interpreted by the physician, who will discuss the findings with the patient and determine any further diagnostic or therapeutic steps that may be necessary based on the results obtained from the ultrasound elastography.

53 Discontinued procedure: under certain circumstances, the physician or other qualified health care professional may elect to terminate a surgical or diagnostic procedure. due to extenuating circumstances or those that threaten the well being of the patient, it may be necessary to indicate that a surgical or diagnostic procedure was started but discontinued. this circumstance may be reported by adding modifier 53 to the code reported by the individual for the discontinued procedure. note: this modifier is not used to report the elective cancellation of a procedure prior to the patient's anesthesia induction and/or surgical preparation in the operating suite. for outpatient hospital/ambulatory surgery center (asc) reporting of a previously scheduled procedure/service that is partially reduced or cancelled as a result of extenuating circumstances or those that threaten the well being of the patient prior to or after administration of anesthesia, see modifiers 73 and 74 (see modifiers approved for asc hospital outpatient use).

77 Repeat procedure by another physician or other qualified health care professional: it may be necessary to indicate that a basic procedure or service was repeated by another physician or other qualified health care professional subsequent to the original procedure or service. this circumstance may be reported by adding modifier 77 to the repeated procedure or service. note: this modifier should not be appended to an e/m service.
95 Synchronous telemedicine service rendered via a real-time interactive audio and video telecommunications system: synchronous telemedicine service is defined as a real-time interaction between a physician or other qualified health care professional and a patient who is located at a distant site from the physician or other qualified health care professional. the totality of the communication of information exchanged between the physician or other qualified health care professional and the patient during the course of the synchronous telemedicine service must be of an amount and nature that would be sufficient to meet the key components and/or requirements of the same service when rendered via a face-to-face interaction. modifier 95 may only be appended to the services listed in appendix p. appendix p is the list of cpt codes for services that are typically performed face-to-face, but may be rendered via a real-time (synchronous) interactive audio and video telecommunications system.
