Common Language Description

Diagnostic mammography, represented by CPT® Code 77065, is a specialized radiographic imaging procedure focused on the breast, utilizing low-dose ionizing radiation to produce detailed images. 1. Indications

Diagnostic mammography (CPT® Code 77065) is indicated for the following conditions:

  • Symptoms of Breast Disease The procedure is performed on patients presenting with symptoms that may indicate breast disease, such as pain, swelling, or changes in breast shape or texture.
  • Palpable Mass It is indicated for women who have a palpable mass in the breast that requires further evaluation to determine its nature.

2. Procedure

The diagnostic mammography procedure involves several key steps to ensure accurate imaging of the breast.

  • Patient Preparation The patient is positioned in front of the mammography machine, and instructions are provided to ensure comfort and cooperation during the procedure. The patient may be asked to remove clothing from the waist up and wear a gown.
  • Breast Compression The breast is placed between two plates of the mammography machine. The machine applies gentle pressure to compress the breast tissue. This compression is essential as it helps to spread out the breast tissue, reducing motion and improving image quality.
  • X-ray Imaging Once the breast is properly positioned and compressed, the radiologic technologist will take X-ray images from different angles. The low-dose ionizing radiation used in mammography allows for detailed visualization of the breast tissue.
  • Computer-Aided Detection (CAD) If CAD is utilized, the obtained images are analyzed using computer algorithms. The system scans the mammographic films, converting them into digital data for further evaluation. The CAD system highlights any areas of concern that may require additional scrutiny by the radiologist.
  • Image Review After the images are captured, they are reviewed by a radiologist who will interpret the findings and determine if further diagnostic steps are necessary.

3. Post-Procedure

After the diagnostic mammography procedure, patients may be advised to resume normal activities immediately. However, they should be informed about potential discomfort due to breast compression, which typically subsides shortly after the procedure. The radiologist will provide a report of the findings, which may take a few days to be available. If any abnormalities are detected, the patient may be referred for additional imaging or biopsy for further evaluation. It is important for patients to follow up with their healthcare provider to discuss the results and any necessary next steps.

59 Distinct procedural service: under certain circumstances, it may be necessary to indicate that a procedure or service was distinct or independent from other non-e/m services performed on the same day. modifier 59 is used to identify procedures/services, other than e/m services, that are not normally reported together, but are appropriate under the circumstances. documentation must support a different session, different procedure or surgery, different site or organ system, separate incision/excision, separate lesion, or separate injury (or area of injury in extensive injuries) not ordinarily encountered or performed on the same day by the same individual. however, when another already established modifier is appropriate it should be used rather than modifier 59. only if no more descriptive modifier is available, and the use of modifier 59 best explains the circumstances, should modifier 59 be used. note: modifier 59 should not be appended to an e/m service. to report a separate and distinct e/m service with a non-e/m service performed on the same date, see modifier 25.
76 Repeat procedure or service by same physician or other qualified health care professional: it may be necessary to indicate that a procedure or service was repeated by the same physician or other qualified health care professional subsequent to the original procedure or service. this circumstance may be reported by adding modifier 76 to the repeated procedure or service. note: this modifier should not be appended to an e/m service.
52 Reduced services: under certain circumstances a service or procedure is partially reduced or eliminated at the discretion of the physician or other qualified health care professional. under these circumstances the service provided can be identified by its usual procedure number and the addition of modifier 52, signifying that the service is reduced. this provides a means of reporting reduced services without disturbing the identification of the basic service. note: for hospital outpatient reporting of a previously scheduled procedure/service that is partially reduced or cancelled as a result of extenuating circumstances or those that threaten the well-being of the patient prior to or after administration of anesthesia, see modifiers 73 and 74 (see modifiers approved for asc hospital outpatient use).
77 Repeat procedure by another physician or other qualified health care professional: it may be necessary to indicate that a basic procedure or service was repeated by another physician or other qualified health care professional subsequent to the original procedure or service. this circumstance may be reported by adding modifier 77 to the repeated procedure or service. note: this modifier should not be appended to an e/m service.

58 Staged or related procedure or service by the same physician or other qualified health care professional during the postoperative period: it may be necessary to indicate that the performance of a procedure or service during the postoperative period was: (a) planned or anticipated (staged); (b) more extensive than the original procedure; or (c) for therapy following a surgical procedure. this circumstance may be reported by adding modifier 58 to the staged or related procedure. note: for treatment of a problem that requires a return to the operating/procedure room (eg, unanticipated clinical condition), see modifier 78.

(for repeat procedures on the same day, see modifier 76.)
99 Multiple modifiers: under certain circumstances 2 or more modifiers may be necessary to completely delineate a service. in such situations modifier 99 should be added to the basic procedure, and other applicable modifiers may be listed as part of the description of the service.
