The procedure described by CPT® Code 11402 involves the excision of a benign lesion from the trunk, arms, or legs, specifically when the excised diameter of the lesion measures between 1.1 to 2.0 cm. A benign lesion is defined as a non-cancerous growth that does not pose a threat to health, and it can include various types of skin abnormalities such as lipomas, dermatofibromas, pyogenic granulomas, epidermoid cysts, and benign nevi. During the procedure, the area surrounding the lesion is first cleansed to reduce the risk of infection, and a local anesthetic is administered to ensure patient comfort. The surgeon then identifies a narrow margin of healthy tissue around the lesion to ensure complete removal. A full-thickness incision is made through the dermis, encircling the lesion, which is then excised in its entirety. The excised tissue is sent to a laboratory for histologic evaluation, which is separately reportable. To manage any bleeding that may occur during the procedure, electrocautery or chemical cautery techniques are employed. After the lesion is removed, the surgical wound may be closed using a simple single-layer suture technique. However, if necessary, more complex closure methods such as intermediate (layer) closure, complex repair, skin grafts, or pedicle flaps may be utilized. This procedure is essential for the removal of benign lesions that may cause discomfort or cosmetic concerns for the patient.

The excision of a benign lesion, as described by CPT® Code 11402, is indicated for the following conditions:
The procedure for excising a benign lesion begins with the preparation of the surgical site. The area surrounding the lesion is thoroughly cleansed to minimize the risk of infection. Following this, a local anesthetic is injected to ensure that the patient remains comfortable throughout the procedure. Once the area is adequately anesthetized, the surgeon identifies a narrow margin of healthy tissue surrounding the lesion. This margin is crucial for ensuring complete excision of the lesion and reducing the likelihood of recurrence. A full-thickness incision is then made through the dermis, carefully encircling the lesion. The incision is designed to allow for the complete removal of the lesion along with the surrounding healthy tissue. After the lesion is excised, it is sent to a laboratory for histologic evaluation, which is a separate reportable service. During the excision, any bleeding that occurs is controlled using electrocautery or chemical cautery techniques. Once the lesion has been successfully removed, the surgical wound may be closed using a simple single-layer suture technique. However, if the wound requires more complex closure, options such as intermediate (layer) closure, complex repair, skin grafts, or pedicle flaps may be employed to ensure optimal healing and cosmetic results.
After the excision of the benign lesion, patients are typically monitored for any immediate complications, such as excessive bleeding or infection. Instructions for post-procedure care are provided, which may include keeping the surgical site clean and dry, monitoring for signs of infection, and managing pain with prescribed medications. Patients are advised to avoid strenuous activities that may stress the surgical site during the initial healing period. Follow-up appointments may be scheduled to assess the healing process and to discuss the results of the histologic evaluation of the excised tissue. It is important for patients to adhere to the post-procedure care instructions to promote optimal recovery and minimize the risk of complications.
