Common Language Description

Coronary artery bypass graft (CABG) is a surgical procedure aimed at improving blood flow to the heart by bypassing blocked or narrowed coronary arteries. This procedure is critical for patients suffering from ischemic heart disease, which can lead to serious conditions such as myocardial infarction (heart attack). The coronary arteries are responsible for delivering oxygen-rich blood to the heart muscle, and any obstruction in these arteries can significantly impair heart function. In CABG, surgeons utilize a combination of venous and arterial grafts to create a new pathway for blood to flow, effectively circumventing the areas of blockage. The procedure typically involves harvesting veins, often the greater saphenous veins from the legs, which are then prepared for grafting. The prepared vein grafts are meticulously sewn into place, connecting one end to the ascending aorta and the other end to the coronary artery beyond the blockage. 1. Indications

The procedure is indicated for patients with significant coronary artery disease characterized by the following conditions:

  • Ischemic Heart Disease - A condition where the heart muscle is deprived of oxygen due to narrowed or blocked coronary arteries.
  • Myocardial Infarction - A heart attack resulting from prolonged ischemia, necessitating restoration of blood flow to prevent further damage to the heart muscle.
  • Severe Angina - Chronic chest pain that occurs due to reduced blood flow to the heart, often leading to decreased quality of life and increased risk of heart complications.
  • Coronary Artery Blockage - Significant narrowing or blockage in one or more coronary arteries that cannot be managed effectively with medication or less invasive procedures.

2. Procedure

The CABG procedure involves several critical steps to ensure successful grafting and restoration of blood flow to the heart:

  • Step 1: Patient Preparation - The patient is positioned on the operating table, and general anesthesia is administered. Monitoring devices are attached to track vital signs throughout the procedure.
  • Step 2: Harvesting Grafts - The surgeon makes incisions to access the greater saphenous veins in the legs or other suitable arteries, such as the internal mammary artery. These vessels are carefully removed and prepared for grafting.
  • Step 3: Chest Incision - A median sternotomy is performed, where the sternum is divided, and the ribs are retracted to provide access to the heart. This allows the surgeon to visualize the coronary arteries clearly.
  • Step 4: Cardiopulmonary Bypass or Off-Pump Technique - Depending on the surgical approach, the heart may be temporarily stopped using cardiopulmonary bypass, or the off-pump technique may be employed, allowing the surgery to be performed on a beating heart after administering medication to slow the heart rate.
  • Step 5: Grafting - The prepared vein grafts are sewn into place. One end of each graft is attached to the ascending aorta, while the other end is connected to the coronary artery beyond the blockage, creating a new pathway for blood flow.
  • Step 6: Closure - After all grafts are in place, the heart is restarted if cardiopulmonary bypass was used. The chest is then closed by wiring the sternum back together and suturing the skin incisions.

3. Post-Procedure

Post-procedure care is essential for recovery and includes monitoring in a cardiac intensive care unit (ICU) for several days. Patients are observed for any complications such as bleeding, infection, or arrhythmias. Pain management is provided, and patients are encouraged to begin light physical activity as soon as they are stable. Cardiac rehabilitation may be recommended to aid recovery and improve cardiovascular health. Follow-up appointments are necessary to assess the success of the grafts and overall heart function.
