Common Language Description

Coronary artery bypass graft (CABG) surgery is a critical procedure aimed at restoring blood flow to the heart by bypassing narrowed or blocked coronary arteries. The coronary arteries are essential as they supply oxygen and nutrients to the heart muscle. When these arteries become narrowed or blocked, it can lead to serious conditions such as ischemic heart disease and myocardial infarction, which can be life-threatening. The CABG procedure involves the use of arterial grafts to reroute blood around the obstructed arteries, thereby improving blood flow to the heart. During the surgery, one or more arteries are harvested from the patient's body to serve as grafts. Commonly used arterial conduits include the internal mammary artery (IMA), which is located in the chest, the inferior epigastric artery (IEA) from the abdomen, and the radial artery from the forearm. In some cases, the gastroepiploic artery or other arterial conduits may also be utilized. The harvested arterial grafts can be prepared as pedicle grafts, which retain surrounding tissues and veins, or as skeletonized grafts, where all surrounding tissues are removed, leaving only the outer layer of the graft. The surgical approach typically involves a median sternotomy to access the thoracic cavity, particularly when the IMA is used. The procedure requires careful dissection and preparation of the grafts, followed by their attachment to the coronary arteries to restore adequate blood flow. The choice of surgical technique, whether off-pump coronary artery bypass (OPCAB) or on-pump using cardiopulmonary bypass, is determined based on the patient's condition and the surgeon's preference. This complex procedure is vital for patients with significant coronary artery disease, aiming to alleviate symptoms and reduce the risk of heart-related complications.

1. Indications

The procedure is indicated for patients with significant coronary artery disease characterized by the following conditions:

  • Ischemic Heart Disease - A condition where the heart muscle is deprived of oxygen due to narrowed or blocked coronary arteries.
  • Myocardial Infarction - A heart attack resulting from the interruption of blood supply to a part of the heart, often due to blockage in the coronary arteries.
  • Severe Angina - Chest pain or discomfort that occurs when the heart does not receive enough blood, often due to coronary artery blockages.
  • Coronary Artery Blockage - Significant narrowing or occlusion of one or more coronary arteries that compromises blood flow to the heart muscle.

2. Procedure

The CABG procedure involves several critical steps to ensure successful grafting of the arterial conduits:

  • Harvesting the Arterial Graft - Prior to accessing the heart, one or more arteries are harvested. The internal mammary artery (IMA) is commonly harvested through a median sternotomy, where the pleura is displaced laterally to access the IMA. The segment to be harvested is identified, and soft tissue incision sites are marked. The IMA is separated from the chest wall at the third or fourth intercostal space, with collateral branches in the distal portion divided. The proximal portion is inspected, and collateral branches are also divided.
  • Initiating Bypass Technique - After harvesting the graft, the surgical team may choose to perform the bypass using either the off-pump coronary artery bypass (OPCAB) technique or the on-pump technique with cardiopulmonary bypass. If OPCAB is selected, the surgery is conducted on the beating heart after administering medication to slow the heart rate. If cardiopulmonary bypass is initiated, the patient is connected to the bypass pump, and cardioplegia is administered to protect the heart during the procedure.
  • Preparing the Graft for Anastomosis - The pericardium is incised, and the aorta is cannulated. The distal end of the IMA is divided, and the graft is rotated. The inferior surface of the IMA is exposed by retracting and incising the endothoracic fascia. The proximal end of the graft is clamped, and the distal end is prepared for anastomosis using a papaverine solution flush and a papaverine-soaked gauze wrap.
  • Attaching the Graft - The prepared arterial graft(s) are sewn into place, with one end attached to the ascending aorta and the other end connected to the diseased coronary artery beyond the blockage. If additional grafts are required, such as the inferior epigastric artery (IEA) or radial artery, they are harvested and prepared similarly, ensuring proper attachment to restore blood flow.

3. Post-Procedure

Post-procedure care involves monitoring the patient for any complications and ensuring proper recovery. If cardiopulmonary bypass was used, it is discontinued, and the patient is gradually weaned off the bypass support. Patients are typically observed in a recovery unit for stabilization before being transferred to a regular hospital room. Pain management, monitoring of vital signs, and assessment of graft function are critical during the initial recovery phase. Patients may also receive instructions on activity restrictions and rehabilitation to promote healing and improve cardiovascular health.

59 Distinct procedural service: under certain circumstances, it may be necessary to indicate that a procedure or service was distinct or independent from other non-e/m services performed on the same day. modifier 59 is used to identify procedures/services, other than e/m services, that are not normally reported together, but are appropriate under the circumstances. documentation must support a different session, different procedure or surgery, different site or organ system, separate incision/excision, separate lesion, or separate injury (or area of injury in extensive injuries) not ordinarily encountered or performed on the same day by the same individual. however, when another already established modifier is appropriate it should be used rather than modifier 59. only if no more descriptive modifier is available, and the use of modifier 59 best explains the circumstances, should modifier 59 be used. note: modifier 59 should not be appended to an e/m service. to report a separate and distinct e/m service with a non-e/m service performed on the same date, see modifier 25.

52 Reduced services: under certain circumstances a service or procedure is partially reduced or eliminated at the discretion of the physician or other qualified health care professional. under these circumstances the service provided can be identified by its usual procedure number and the addition of modifier 52, signifying that the service is reduced. this provides a means of reporting reduced services without disturbing the identification of the basic service. note: for hospital outpatient reporting of a previously scheduled procedure/service that is partially reduced or cancelled as a result of extenuating circumstances or those that threaten the well-being of the patient prior to or after administration of anesthesia, see modifiers 73 and 74 (see modifiers approved for asc hospital outpatient use).
53 Discontinued procedure: under certain circumstances, the physician or other qualified health care professional may elect to terminate a surgical or diagnostic procedure. due to extenuating circumstances or those that threaten the well being of the patient, it may be necessary to indicate that a surgical or diagnostic procedure was started but discontinued. this circumstance may be reported by adding modifier 53 to the code reported by the individual for the discontinued procedure. note: this modifier is not used to report the elective cancellation of a procedure prior to the patient's anesthesia induction and/or surgical preparation in the operating suite. for outpatient hospital/ambulatory surgery center (asc) reporting of a previously scheduled procedure/service that is partially reduced or cancelled as a result of extenuating circumstances or those that threaten the well being of the patient prior to or after administration of anesthesia, see modifiers 73 and 74 (see modifiers approved for asc hospital outpatient use).
55 Postoperative management only: when 1 physician or other qualified health care professional performed the postoperative management and another performed the surgical procedure, the postoperative component may be identified by adding modifier 55 to the usual procedure number.
58 Staged or related procedure or service by the same physician or other qualified health care professional during the postoperative period: it may be necessary to indicate that the performance of a procedure or service during the postoperative period was: (a) planned or anticipated (staged); (b) more extensive than the original procedure; or (c) for therapy following a surgical procedure. this circumstance may be reported by adding modifier 58 to the staged or related procedure. note: for treatment of a problem that requires a return to the operating/procedure room (eg, unanticipated clinical condition), see modifier 78.
62 Two surgeons: when 2 surgeons work together as primary surgeons performing distinct part(s) of a procedure, each surgeon should report his/her distinct operative work by adding modifier 62 to the procedure code and any associated add-on code(s) for that procedure as long as both surgeons continue to work together as primary surgeons. each surgeon should report the co-surgery once using the same procedure code. if additional procedure(s) (including add-on procedure(s) are performed during the same surgical session, separate code(s) may also be reported with modifier 62 added. note: if a co-surgeon acts as an assistant in the performance of additional procedure(s), other than those reported with the modifier 62, during the same surgical session, those services may be reported using separate procedure code(s) with modifier 80 or modifier 82 added, as appropriate.
