Official Description

Open iliac artery exposure with creation of conduit for delivery of endovascular prosthesis or for establishment of cardiopulmonary bypass, by abdominal or retroperitoneal incision, unilateral (List separately in addition to code for primary procedure)

Common Language Description

The procedure described by CPT® Code 34833 involves the surgical exposure of the iliac artery, which is a major blood vessel located in the pelvis. This exposure is achieved through an abdominal or retroperitoneal incision, allowing the surgeon to create a conduit. The conduit serves two primary purposes: it facilitates the delivery of an endovascular prosthesis for the repair of aortic or iliac defects, or it establishes a pathway for cardiopulmonary bypass. The creation of a conduit is particularly important when direct cannulation of the iliac artery is not advisable due to anatomical considerations or the patient's condition. During the procedure, the surgeon carefully incises the skin in the lower abdomen or retroperitoneum and meticulously dissects through the tissue to access the iliac artery, ensuring the protection of surrounding structures such as veins, nerves, and the ureter. The procedure requires precise dissection of arterial branches, with bleeding controlled through ligation or electrocautery. The iliac artery is then freed from surrounding tissue over a length of 5-6 cm and secured with vessel loops to maintain its position. Following this, the artery is clamped and incised to allow for the tailored conduit to be anastomosed to it. This conduit may be utilized for various surgical interventions, including the establishment of cardiopulmonary bypass, which is critical in certain cardiac procedures. The entire process is performed unilaterally and is reported separately in addition to the primary procedure, highlighting its significance in complex vascular surgeries.

1. Indications

The procedure described by CPT® Code 34833 is indicated for specific clinical scenarios where access to the iliac artery is necessary for further surgical intervention. The following conditions may warrant this procedure:

  • Endovascular Prosthesis Delivery The creation of a conduit is essential for the delivery of an endovascular prosthesis, particularly in cases of aortic or iliac artery defects that require repair.
  • Cardiopulmonary Bypass Establishment This procedure is indicated when there is a need to establish cardiopulmonary bypass, which may be necessary during complex cardiac surgeries or when direct access to the heart is required.

2. Procedure

The procedure involves several critical steps to ensure successful exposure and conduit creation for the iliac artery:

  • Step 1: Incision The surgeon begins by making an incision in the lower abdomen or retroperitoneum to access the iliac artery. This incision is carefully planned to minimize trauma to surrounding tissues.
  • Step 2: Tissue Dissection Following the incision, the surgeon meticulously dissects through the tissue layers to reach the iliac artery. During this dissection, care is taken to protect adjacent structures, including veins, nerves, and the ureter, to prevent complications.
  • Step 3: Arterial Exposure Once the iliac artery is located, the surgeon dissects it free from surrounding tissue over a length of 5-6 cm. This exposure is crucial for the subsequent steps and is secured using vessel loops to maintain visibility and access.
  • Step 4: Clamping and Incision The iliac artery is then clamped to control blood flow, and an incision is made in the artery to facilitate the anastomosis of the conduit.
  • Step 5: Conduit Selection and Anastomosis A conduit is selected and tailored to fit the specific anatomical requirements. The conduit is then anastomosed to the iliac artery, ensuring a secure connection for either the endovascular prosthesis or cardiopulmonary bypass.
  • Step 6: Clamping and Leak Control After the conduit is in place, it is clamped, and the previously placed clamps on the iliac artery are removed. Any vascular leaks at the anastomosis site are reinforced with sutures to ensure hemostasis.
  • Step 7: Cardiopulmonary Bypass Setup (if applicable) If the conduit is created for cardiopulmonary bypass, the graft conduit is sewn to the iliac artery after administering heparin. An arterial cannula is then connected, and a venous cannula is placed either peripherally or centrally to establish bypass.
  • Step 8: Finalization Upon completion of the procedure or placement of the endovascular prosthesis, the graft conduit may either be left in place with the distal end anastomosed to the iliac artery or cut short, with the stump closed using sutures.

3. Post-Procedure

Post-procedure care involves monitoring the patient for any complications related to the surgical site, including bleeding or infection. The patient may require follow-up imaging to assess the integrity of the conduit and the success of the anastomosis. Recovery protocols will depend on the extent of the procedure and the patient's overall health status. It is essential to provide appropriate postoperative instructions regarding activity restrictions and signs of potential complications that should prompt immediate medical attention.

58 Staged or related procedure or service by the same physician or other qualified health care professional during the postoperative period: it may be necessary to indicate that the performance of a procedure or service during the postoperative period was: (a) planned or anticipated (staged); (b) more extensive than the original procedure; or (c) for therapy following a surgical procedure. this circumstance may be reported by adding modifier 58 to the staged or related procedure. note: for treatment of a problem that requires a return to the operating/procedure room (eg, unanticipated clinical condition), see modifier 78.
59 Distinct procedural service: under certain circumstances, it may be necessary to indicate that a procedure or service was distinct or independent from other non-e/m services performed on the same day. modifier 59 is used to identify procedures/services, other than e/m services, that are not normally reported together, but are appropriate under the circumstances. documentation must support a different session, different procedure or surgery, different site or organ system, separate incision/excision, separate lesion, or separate injury (or area of injury in extensive injuries) not ordinarily encountered or performed on the same day by the same individual. however, when another already established modifier is appropriate it should be used rather than modifier 59. only if no more descriptive modifier is available, and the use of modifier 59 best explains the circumstances, should modifier 59 be used. note: modifier 59 should not be appended to an e/m service. to report a separate and distinct e/m service with a non-e/m service performed on the same date, see modifier 25.
62 Two surgeons: when 2 surgeons work together as primary surgeons performing distinct part(s) of a procedure, each surgeon should report his/her distinct operative work by adding modifier 62 to the procedure code and any associated add-on code(s) for that procedure as long as both surgeons continue to work together as primary surgeons. each surgeon should report the co-surgery once using the same procedure code. if additional procedure(s) (including add-on procedure(s) are performed during the same surgical session, separate code(s) may also be reported with modifier 62 added. note: if a co-surgeon acts as an assistant in the performance of additional procedure(s), other than those reported with the modifier 62, during the same surgical session, those services may be reported using separate procedure code(s) with modifier 80 or modifier 82 added, as appropriate.
80 Assistant surgeon: surgical assistant services may be identified by adding modifier 80 to the usual procedure number(s).
AS Physician assistant, nurse practitioner, or clinical nurse specialist services for assistant at surgery
GC This service has been performed in part by a resident under the direction of a teaching physician
LT Left side (used to identify procedures performed on the left side of the body)
