1. Indications

The endovascular repair of the infrarenal aorta using an aorto-aortic tube endograft is indicated for several critical conditions, including:

  • Aneurysm A localized enlargement of the aorta that can lead to rupture if not treated.
  • Pseudoaneurysm A false aneurysm that occurs when blood leaks out of the artery but is contained by surrounding tissue.
  • Dissection A serious condition where there is a tear in the inner layer of the aorta, leading to separation of the layers of the aortic wall.
  • Pentrating ulcer An ulcer that penetrates the aortic wall, which can lead to serious complications including rupture.
  • Traumatic disruption Injury to the aorta that can occur due to blunt or penetrating trauma, necessitating urgent repair.

2. Procedure

The procedure for endovascular repair of the infrarenal aorta involves several critical steps:

  • Step 1: Pre-procedure sizing and device selection are performed to ensure the appropriate fit of the endograft for the patient's anatomy. This is crucial for the success of the procedure.
  • Step 2: Access is gained to the abdominal aorta through the femoral artery, where a guidewire is introduced. This guidewire serves as a pathway for the subsequent placement of the endograft.
  • Step 3: A sheath containing the endograft is advanced over the guidewire and positioned below the renal arteries, utilizing fluoroscopic guidance to ensure accurate placement.
  • Step 4: The tube graft, made of non-permeable polyester and supported by a self-expanding metal frame, is partially deployed. The upper portion of the device is positioned just below the renal arteries.
  • Step 5: A balloon is introduced and inflated to expand the graft against the vessel wall, ensuring a tight seal from proximal to distal.
  • Step 6: After achieving the desired expansion, the sheath and guidewire are removed, and the femoral artery is repaired.
  • Step 7: In cases of rupture, two lines are established in the aorta from both the right and left femoral arteries. Guidewires and balloons are interchanged to occlude the hemorrhage and place a supportive stent.
  • Step 8: A large balloon is introduced over a guidewire to occlude the aorta, followed by the deployment of a supportive stent.
  • Step 9: A new occlusion balloon is placed from the ipsilateral femoral artery to the stent graft, and the first balloon is deflated and removed.
  • Step 10: The main aortic tube endograft is deployed and positioned below the renal arteries, followed by the deflation and removal of the second balloon.
  • Step 11: A balloon is introduced inside the endograft to expand it against the vessel wall, ensuring a secure fit.
  • Step 12: Finally, the sheath and guidewires are removed, and the femoral arteries are repaired to complete the procedure.

3. Post-Procedure

Post-procedure care involves monitoring the patient for any complications, such as bleeding or infection at the access site. Patients may require imaging studies to assess the integrity of the endograft and ensure that there are no leaks or other issues. Recovery typically involves a short hospital stay, with gradual resumption of normal activities as advised by the healthcare provider. Follow-up appointments are essential to monitor the patient's progress and the performance of the endograft over time.
