Common Language Description

The procedure described by CPT® Code 34715 involves the surgical exposure of the axillary or subclavian artery through an incision made either below (infraclavicular) or above (supraclavicular) the clavicle. This exposure is essential for the delivery of an endovascular prosthesis, which is a device used to treat vascular conditions. The procedure may also be performed to facilitate cardiopulmonary bypass, a technique that temporarily takes over the function of the heart and lungs during surgery. The process begins with a careful incision in the skin, followed by meticulous dissection of the surrounding tissues to access the artery while protecting adjacent veins and nerves. The surgeon dissects the arterial branches and controls any bleeding that may occur using ligation or electrocautery. 1. Indications

The procedure described by CPT® Code 34715 is indicated for specific clinical scenarios where access to the axillary or subclavian artery is necessary. The following conditions may warrant this procedure:

  • Delivery of Endovascular Prosthesis This procedure is performed to facilitate the placement of an endovascular prosthesis, which is used to treat various vascular diseases.
  • Cardiopulmonary Bypass The exposure of the axillary/subclavian artery may also be indicated for establishing cardiopulmonary bypass during surgical interventions that require temporary support of heart and lung function.

2. Procedure

The procedure involves several critical steps to ensure proper exposure and access to the axillary or subclavian artery:

  • Step 1: Incision The surgeon begins by making an incision in the skin either above or below the clavicle, depending on the approach chosen (supraclavicular or infraclavicular). This incision allows access to the underlying tissues and the artery.
  • Step 2: Tissue Dissection Following the incision, the surgeon carefully dissects the surrounding tissues to expose the axillary or subclavian artery. During this process, great care is taken to protect nearby veins and nerves to prevent any potential damage.
  • Step 3: Control of Bleeding As the dissection progresses, any bleeding that occurs is controlled using ligation or electrocautery techniques. This step is crucial to maintain a clear surgical field and ensure patient safety.
  • Step 4: Artery Exposure The axillary or subclavian artery is dissected free from surrounding tissues over a length of approximately 5-6 cm. The artery is then secured in place using vessel loops to facilitate further manipulation.
  • Step 5: Clamping and Incision Once adequately exposed, the artery is clamped to prevent blood flow, and an incision is made in the artery to allow for the insertion of a conduit.
  • Step 6: Conduit Selection and Anastomosis A conduit is selected and tailored to fit the specific requirements of the procedure. This conduit is then anastomosed to the axillary or subclavian artery, ensuring a secure connection.
  • Step 7: Clamping and Leak Control After the conduit is in place, it is clamped. The previously placed clamps on the artery are then removed, and any vascular leaks at the anastomosis site are reinforced with sutures to ensure hemostasis.
  • Step 8: Cardiopulmonary Bypass Setup (if applicable) If the conduit is created for cardiopulmonary bypass, the graft conduit is sewn to the axillary or subclavian artery after administering heparin. An arterial cannula is then connected, and a venous cannula may be placed peripherally or centrally to establish bypass.
  • Step 9: Final Steps Following the completion of the procedure or the placement of the endovascular prosthesis, the graft conduit may either be left in place with the distal end anastomosed to the axillary/subclavian artery or cut short, with the stump closed using sutures.

3. Post-Procedure

After the completion of the procedure, patients may require monitoring for any complications related to the surgical site, such as bleeding or infection. The recovery process will depend on the extent of the surgery and the patient's overall health. If a conduit was created for cardiopulmonary bypass, additional monitoring may be necessary to assess the function of the bypass and the patient's hemodynamic status. Follow-up care will typically include assessing the surgical site for proper healing and ensuring that the vascular access remains patent. Any specific post-operative instructions will be provided by the surgical team to ensure optimal recovery.
