Common Language Description

A flexible sigmoidoscopy is a diagnostic and therapeutic procedure that involves the examination of the sigmoid colon using a flexible endoscope. This procedure is specifically performed with the placement of an endoscopic stent, which is a tube-like device used to keep a narrowed segment of the colon open. The process begins with the introduction of a standard flexible sigmoidoscope into the anus, which is then advanced through the rectum and into the sigmoid colon. During this advancement, air insufflation is utilized to separate the mucosal folds, enhancing the visualization of the colon's inner lining. Once the scope reaches the area of stenosis, the physician assesses the position and length of the narrowing. If the examination indicates that pre-dilation is necessary to facilitate stent placement, a guidewire is inserted through the scope. Following this, a series of rigid tubes with increasing diameters may be passed over the guidewire to dilate the stricture, or alternatively, a balloon catheter may be used to inflate the stricture directly. A guidewire is again passed through the scope, followed by the stent delivery system, which is carefully positioned in the narrowed segment of the colon. The appropriate size of the stent is selected and introduced through the scope, where it is then deployed or expanded within the stricture. To ensure the stent is correctly positioned and properly deployed, a balloon catheter may be inserted and inflated to secure the stent in place. Finally, the endoscope is advanced through the stent to verify its correct positioning before the scope is withdrawn, completing the procedure.

1. Indications

The flexible sigmoidoscopy with endoscopic stent placement is indicated for various conditions that result in a narrowed segment of the colon. These indications may include:

  • Colonic Strictures - Narrowing of the colon due to conditions such as inflammatory bowel disease, cancer, or previous surgical interventions.
  • Obstruction - Situations where the flow of intestinal contents is impeded, necessitating the placement of a stent to alleviate the blockage.
  • Benign Tumors - Non-cancerous growths that may cause narrowing and require stenting for symptomatic relief.

2. Procedure

The procedure involves several critical steps to ensure successful stent placement. Each step is designed to facilitate the examination and treatment of the narrowed segment of the colon.

  • Step 1: Introduction of the Sigmoidoscope - The procedure begins with the careful introduction of a flexible sigmoidoscope into the anus. The scope is advanced through the rectum and into the sigmoid colon, utilizing air insufflation to separate the mucosal folds. This technique enhances visualization of the mucosal surface, allowing for a thorough examination of the colon.
  • Step 2: Assessment of Stenosis - Once the scope is positioned at the area of stenosis, the physician evaluates the position and length of the narrowing. This assessment is crucial for determining the appropriate course of action, including whether pre-dilation is necessary.
  • Step 3: Pre-Dilation (if needed) - If the examination indicates that pre-dilation is required, a guidewire is inserted through the scope. A series of rigid tubes with increasing diameters may be passed over the guidewire to dilate the stricture. Alternatively, a balloon catheter may be advanced to the site of the stricture and inflated to achieve the necessary dilation.
  • Step 4: Stent Placement - Following successful dilation, the stent placement procedure begins. An appropriately sized stent is selected and introduced through the scope.
  • Step 5: Deployment of the Stent - The stent is positioned in the narrowed segment of the colon and deployed, or expanded, to maintain patency. To ensure proper seating of the stent, a balloon catheter may be inserted and inflated.
  • Step 6: Verification of Stent Position - Finally, the endoscope is advanced through the stent to verify its correct position and ensure that it has been properly deployed. Once confirmed, the scope is withdrawn, completing the procedure.

3. Post-Procedure

After the procedure, patients may be monitored for any immediate complications or adverse effects. It is essential to assess the patient's recovery and ensure that they are stable before discharge. Follow-up care may include instructions on dietary modifications, signs of complications to watch for, and scheduling any necessary follow-up appointments to evaluate the effectiveness of the stent placement and the overall condition of the colon.

52 Reduced services: under certain circumstances a service or procedure is partially reduced or eliminated at the discretion of the physician or other qualified health care professional. under these circumstances the service provided can be identified by its usual procedure number and the addition of modifier 52, signifying that the service is reduced. this provides a means of reporting reduced services without disturbing the identification of the basic service. note: for hospital outpatient reporting of a previously scheduled procedure/service that is partially reduced or cancelled as a result of extenuating circumstances or those that threaten the well-being of the patient prior to or after administration of anesthesia, see modifiers 73 and 74 (see modifiers approved for asc hospital outpatient use).
53 Discontinued procedure: under certain circumstances, the physician or other qualified health care professional may elect to terminate a surgical or diagnostic procedure. due to extenuating circumstances or those that threaten the well being of the patient, it may be necessary to indicate that a surgical or diagnostic procedure was started but discontinued. this circumstance may be reported by adding modifier 53 to the code reported by the individual for the discontinued procedure. note: this modifier is not used to report the elective cancellation of a procedure prior to the patient's anesthesia induction and/or surgical preparation in the operating suite. for outpatient hospital/ambulatory surgery center (asc) reporting of a previously scheduled procedure/service that is partially reduced or cancelled as a result of extenuating circumstances or those that threaten the well being of the patient prior to or after administration of anesthesia, see modifiers 73 and 74 (see modifiers approved for asc hospital outpatient use).
58 Staged or related procedure or service by the same physician or other qualified health care professional during the postoperative period: it may be necessary to indicate that the performance of a procedure or service during the postoperative period was: (a) planned or anticipated (staged); (b) more extensive than the original procedure; or (c) for therapy following a surgical procedure. this circumstance may be reported by adding modifier 58 to the staged or related procedure. note: for treatment of a problem that requires a return to the operating/procedure room (eg, unanticipated clinical condition), see modifier 78.
59 Distinct procedural service: under certain circumstances, it may be necessary to indicate that a procedure or service was distinct or independent from other non-e/m services performed on the same day. modifier 59 is used to identify procedures/services, other than e/m services, that are not normally reported together, but are appropriate under the circumstances. documentation must support a different session, different procedure or surgery, different site or organ system, separate incision/excision, separate lesion, or separate injury (or area of injury in extensive injuries) not ordinarily encountered or performed on the same day by the same individual. however, when another already established modifier is appropriate it should be used rather than modifier 59. only if no more descriptive modifier is available, and the use of modifier 59 best explains the circumstances, should modifier 59 be used. note: modifier 59 should not be appended to an e/m service. to report a separate and distinct e/m service with a non-e/m service performed on the same date, see modifier 25.
79 Unrelated procedure or service by the same physician or other qualified health care professional during the postoperative period: the individual may need to indicate that the performance of a procedure or service during the postoperative period was unrelated to the original procedure. this circumstance may be reported by using modifier 79. (for repeat procedures on the same day, see modifier 76.)
