Common Language Description

Computed tomography (CT), also known as a CT scan, is a diagnostic imaging procedure that utilizes advanced X-ray technology and computer processing to create detailed cross-sectional images of specific areas within the body. In the case of CPT® Code 70487, the focus is on the maxillofacial area, which encompasses critical structures such as the forehead, sinuses, nose, nasal bones, and the upper and lower jaws (maxilla and mandible). It is important to note that the orbit, which includes the eye socket, is not part of this imaging study. During the procedure, the patient is carefully positioned on a CT examination table, and an initial scan is conducted to establish the starting point for the imaging process. Following this, the CT scan is executed as the table gradually moves through the scanner. This movement allows multiple X-ray beams and electronic detectors to rotate around the targeted body region, measuring the radiation absorption and capturing detailed images. The data collected is then processed by a computer program, resulting in 2D cross-sectional images that are displayed on a monitor for the physician's review. The physician can assess these images in real-time and may request additional scans to focus on specific areas of interest. The use of intravenous contrast material is a key aspect of this procedure, as it enhances the visibility of structures and abnormalities within the maxillofacial area, leading to more accurate diagnoses. For procedures without contrast, CPT® Code 70486 is applicable, while CPT® Code 70488 is used when a CT scan is first performed without contrast, followed by the administration of contrast for further imaging. Ultimately, the physician interprets the findings from the CT scan, documenting any abnormalities in a written report.

1. Indications

The procedure described by CPT® Code 70487 is indicated for various clinical scenarios where detailed imaging of the maxillofacial area is necessary. The following conditions may warrant the use of this CT scan:

  • Facial Trauma Assessment of injuries to the facial bones, including fractures of the maxilla, mandible, and nasal bones.
  • Sinus Disease Evaluation of chronic sinusitis or other sinus-related conditions that may affect the maxillofacial region.
  • Neoplasms Detection and characterization of tumors or lesions within the maxillofacial area.
  • Infections Identification of abscesses or other infectious processes affecting the facial structures.
  • Preoperative Planning Assistance in surgical planning for procedures involving the maxillofacial region, such as reconstructive surgery or dental implants.

2. Procedure

The procedure for CPT® Code 70487 involves several key steps to ensure accurate imaging of the maxillofacial area. The following outlines the procedural steps:

  • Patient Positioning The patient is positioned comfortably on the CT examination table, ensuring that the maxillofacial area is properly aligned with the scanner for optimal imaging.
  • Initial Scan An initial pass through the CT scanner is performed to determine the starting position for the subsequent scans. This step is crucial for establishing the correct imaging parameters.
  • Administration of Contrast Material Intravenous contrast material is administered to enhance the visibility of the structures within the maxillofacial area. This contrast helps to delineate blood vessels and highlight abnormalities.
  • CT Scanning As the table moves slowly through the scanner, multiple X-ray beams and electronic detectors rotate around the maxillofacial region. The system measures the amount of radiation absorbed by the tissues, capturing detailed images.
  • Data Processing The data collected during the scanning process is processed by a computer program, which generates 2D cross-sectional images of the maxillofacial area. These images are displayed on a monitor for immediate review.
  • Image Review The physician reviews the images in real-time, assessing for any abnormalities. If necessary, additional sections may be requested to provide further detail on areas of interest.

3. Post-Procedure

After the completion of the CT scan using CPT® Code 70487, the patient may be monitored briefly to ensure there are no immediate adverse reactions to the contrast material. The physician will interpret the images obtained during the procedure, documenting any findings in a written report. Patients are typically advised to drink plenty of fluids post-procedure to help flush the contrast material from their system. Any specific post-procedure care instructions will be provided based on the individual patient's condition and the findings from the CT scan.

0722T Add On Code MPFS Status: Carrier Priced APC S Quantitative computed tomography (CT) tissue characterization, including interpretation and report, obtained with concurrent CT examination of any structure contained in the concurrently acquired diagnostic imaging dataset (List separately in addition to code for primary procedure)
26 Professional component: certain procedures are a combination of a physician or other qualified health care professional component and a technical component. when the physician or other qualified health care professional component is reported separately, the service may be identified by adding modifier 26 to the usual procedure number.
X5 Diagnostic services requested by another clinician: for reporting services by a clinician who furnishes care to the patient only as requested by another clinician or subsequent and related services requested by another clinician; this modifier is reported for patient relationships that may not be adequately captured by the above alternative categories; reporting clinician service examples include but are not limited to, the radiologist's interpretation of an imaging study requested by another clinician
51 Multiple procedures: when multiple procedures, other than e/m services, physical medicine and rehabilitation services or provision of supplies (eg, vaccines), are performed at the same session by the same individual, the primary procedure or service may be reported as listed. the additional procedure(s) or service(s) may be identified by appending modifier 51 to the additional procedure or service code(s). note: this modifier should not be appended to designated "add-on" codes (see appendix d).
MG The order for this service does not have applicable appropriate use criteria in the qualified clinical decision support mechanism consulted by the ordering professional
GC This service has been performed in part by a resident under the direction of a teaching physician
MH Unknown if ordering professional consulted a clinical decision support mechanism for this service, related information was not provided to the furnishing professional or provider
MA Ordering professional is not required to consult a clinical decision support mechanism due to service being rendered to a patient with a suspected or confirmed emergency medical condition
TC Technical component; under certain circumstances, a charge may be made for the technical component alone; under those circumstances the technical component charge is identified by adding modifier 'tc' to the usual procedure number; technical component charges are institutional charges and not billed separately by physicians; however, portable x-ray suppliers only bill for technical component and should utilize modifier tc; the charge data from portable x-ray suppliers will then be used to build customary and prevailing profiles
MF The order for this service does not adhere to the appropriate use criteria in the clinical decision support mechanism consulted by the ordering professional
