Common Language Description

Magnetic resonance imaging (MRI) is a sophisticated imaging technique utilized to visualize the internal structures of the body, particularly the joints of the upper extremity, such as the shoulder, elbow, wrist, and hand. This noninvasive procedure employs powerful magnets and radio waves to generate detailed images without the use of ionizing radiation, making it a safer alternative to other imaging modalities like X-rays or CT scans. During the MRI process, the patient is positioned on a motorized table that moves into a large cylindrical scanner, which houses the magnet. The strong magnetic field produced by the scanner aligns the hydrogen atoms present in the body. Subsequently, radiofrequency pulses are applied, causing these aligned protons to emit signals that are captured and processed by a computer to create high-resolution, three-dimensional images of the joint in question. The use of contrast material, specifically iodine-based contrast dye, enhances the visibility of the joint structures, allowing for a more precise evaluation of abnormalities. This is particularly beneficial in diagnosing conditions related to injury, trauma, unexplained pain, swelling, or loss of motion in the joint. MRI is adept at revealing critical information regarding the presence of tumors, lesions, or masses, as well as assessing inflammation, infection, and other soft tissue conditions. The detailed images produced by MRI are invaluable for healthcare professionals in correlating the findings with the patient's clinical symptoms, thereby aiding in accurate diagnosis and treatment planning.

1. Indications

Magnetic resonance imaging (MRI) of the upper extremity joint with contrast material is indicated for various clinical scenarios. The following conditions or symptoms may warrant the use of this imaging technique:

  • Injury or Trauma - MRI is often performed to assess damage to the joint structures following an injury or trauma, providing insights into potential tears, fractures, or other injuries.
  • Unexplained Pain - When a patient presents with persistent pain in the joint that cannot be attributed to a clear cause, MRI can help identify underlying issues.
  • Redness or Swelling - The presence of unexplained redness or swelling around the joint may indicate inflammation or other pathological conditions, making MRI a useful diagnostic tool.
  • Loss of Motion - Patients experiencing a freezing of the joint or a significant reduction in range of motion may require MRI to evaluate the underlying causes.

2. Procedure

The procedure for conducting an MRI of the upper extremity joint with contrast material involves several key steps to ensure accurate imaging and patient safety:

  • Patient Preparation - Prior to the MRI, the patient is screened for any contraindications to the procedure, such as the presence of metal implants or allergies to contrast material. The patient may be asked to change into a gown and remove any metallic objects.
  • Administration of Contrast Material - An iodine-based contrast dye is administered, typically through an intravenous line or directly into the joint space, to enhance the visibility of the joint structures during imaging.
  • Positioning the Patient - The patient is positioned on a motorized table that slides into the MRI scanner. Care is taken to ensure that the joint of interest is centered within the magnetic field for optimal imaging.
  • Imaging Process - Once the patient is in position, the MRI machine is activated. The powerful magnetic field and radio waves are used to capture images of the joint. The patient may hear loud tapping or thumping noises during the scan, which is normal.
  • Image Acquisition - Multiple images are taken from various angles to provide a comprehensive view of the joint. The use of contrast material allows for better differentiation of tissues and identification of abnormalities.
  • Post-Procedure Monitoring - After the imaging is complete, the patient is monitored briefly to ensure there are no adverse reactions to the contrast material. The patient can typically resume normal activities immediately following the procedure.

3. Post-Procedure

Following the MRI with contrast, patients are generally advised to drink plenty of fluids to help flush the contrast material from their system. They should be monitored for any allergic reactions to the contrast dye, although such reactions are rare. The physician will review the images obtained during the procedure to assess for any abnormalities and correlate the findings with the patient's clinical symptoms. The results will be discussed with the patient in a follow-up appointment, where further management or treatment options may be considered based on the findings.

53 Discontinued procedure: under certain circumstances, the physician or other qualified health care professional may elect to terminate a surgical or diagnostic procedure. due to extenuating circumstances or those that threaten the well being of the patient, it may be necessary to indicate that a surgical or diagnostic procedure was started but discontinued. this circumstance may be reported by adding modifier 53 to the code reported by the individual for the discontinued procedure. note: this modifier is not used to report the elective cancellation of a procedure prior to the patient's anesthesia induction and/or surgical preparation in the operating suite. for outpatient hospital/ambulatory surgery center (asc) reporting of a previously scheduled procedure/service that is partially reduced or cancelled as a result of extenuating circumstances or those that threaten the well being of the patient prior to or after administration of anesthesia, see modifiers 73 and 74 (see modifiers approved for asc hospital outpatient use).
59 Distinct procedural service: under certain circumstances, it may be necessary to indicate that a procedure or service was distinct or independent from other non-e/m services performed on the same day. modifier 59 is used to identify procedures/services, other than e/m services, that are not normally reported together, but are appropriate under the circumstances. documentation must support a different session, different procedure or surgery, different site or organ system, separate incision/excision, separate lesion, or separate injury (or area of injury in extensive injuries) not ordinarily encountered or performed on the same day by the same individual. however, when another already established modifier is appropriate it should be used rather than modifier 59. only if no more descriptive modifier is available, and the use of modifier 59 best explains the circumstances, should modifier 59 be used. note: modifier 59 should not be appended to an e/m service. to report a separate and distinct e/m service with a non-e/m service performed on the same date, see modifier 25.
