Common Language Description

Acoustic cardiography is an advanced diagnostic technology utilized primarily in the evaluation and management of patients with heart failure, as well as those undergoing cardiac resynchronization therapy (CRT) optimization. This innovative procedure integrates the simultaneous recording and analysis of electrocardiogram (ECG) data alongside heart sound data, providing a comprehensive assessment of cardiac function. During the procedure, two single-channel sound sensors are strategically placed in the V3 and V4 positions on the chest, while two ECG electrodes are also positioned to capture a single-channel ECG, which serves as a timing reference. The acoustic cardiography device is then activated to record both sound and electrical activity over a 10-second interval. The recorded data undergoes automated analysis, with results displayed on a computer monitor for further evaluation. This technology is particularly valuable for assessing left ventricular function, filling patterns, pressures, and ejection fractions, thereby offering insights into the hemodynamic status of the heart. Additionally, acoustic cardiography is instrumental in monitoring cardiac dyssynchrony during CRT, allowing for the evaluation of atrioventricular (AV) and interventricular (VV) delays. For patients receiving CRT, the device can be reprogrammed to adjust delay settings as needed. It is important to note that CPT® Code 0224T is designated for multiple studies that include limited reprogramming of device parameters and evaluation of AV or VV delays, while CPT® Code 0223T is used for a single study, and CPT® Code 0225T is applicable for multiple studies with evaluation of both AV and VV delays. Each of these procedures encompasses a physician's interpretation and a written report detailing the findings.

1. Indications

Acoustic cardiography is indicated for the following conditions and scenarios:

  • Heart Failure This technology is utilized in the diagnosis and management of patients suffering from heart failure, providing critical insights into cardiac function.
  • Cardiac Resynchronization Therapy (CRT) Optimization Acoustic cardiography is employed to optimize CRT in patients, allowing for the assessment of cardiac dyssynchrony and the adjustment of device parameters.

2. Procedure

The procedure for acoustic cardiography involves several key steps that ensure accurate data collection and analysis:

  • Step 1: Patient Preparation The patient is positioned comfortably, and the skin is prepared for electrode placement. Two single-channel sound sensors are placed in the V3 and V4 positions on the chest to capture heart sounds, while two ECG electrodes are positioned to record a single-channel ECG for timing purposes.
  • Step 2: Device Activation Once the sensors and electrodes are securely placed, the acoustic cardiography device is activated. This device is designed to simultaneously record both the heart sounds and the electrical activity of the heart.
  • Step 3: Data Recording The device records the sound and electrical activity for a duration of 10 seconds. This interval is crucial for capturing the necessary data to analyze the heart's function accurately.
  • Step 4: Automated Analysis After the recording period, the data is automatically analyzed by the device. The analysis includes evaluating the heart's hemodynamic function, such as left ventricular function, filling patterns, pressures, and ejection fractions.
  • Step 5: Display of Results The results of the analysis are displayed on a computer monitor, allowing healthcare professionals to review the findings in real-time.
  • Step 6: Reprogramming (if applicable) For patients undergoing CRT, the device may be reprogrammed to adjust AV or VV delay settings based on the analysis results, facilitating optimal therapy.
  • Step 7: Interpretation and Reporting Finally, a physician interprets the results and generates a written report detailing the findings, which is essential for ongoing patient management and treatment planning.

3. Post-Procedure

Post-procedure care for patients who have undergone acoustic cardiography typically involves monitoring for any immediate reactions to the procedure. Patients may be advised to resume normal activities unless otherwise directed by their healthcare provider. The physician will review the written report generated from the procedure, which includes the interpretation of the data collected. Follow-up appointments may be scheduled to discuss the findings and any necessary adjustments to treatment plans, particularly for those undergoing CRT. It is essential for healthcare providers to ensure that patients understand the results and implications of the acoustic cardiography analysis for their ongoing care.
