Official Description

Revision or removal of pulse generator or electrodes, including imaging guidance, when performed, including addition of new electrodes, when performed

Common Language Description

The procedure described by CPT® Code 0284T involves the revision or removal of a pulse generator or electrodes associated with a permanent subcutaneous neurostimulator. This procedure may include imaging guidance if necessary. The primary goal is to address issues related to the implantable pulse generator (IPG) and the electrode arrays that deliver electrical impulses to manage pain. During the revision process, the existing incision over the subcutaneous pocket is reopened to access the IPG and the distal ends of the electrode wires. The connections between the wires and the IPG are carefully examined to ensure proper functionality, and any necessary adjustments or revisions are made. If the electrode arrays are found to be ineffective in delivering impulses to the targeted area of pain, they can be repositioned or replaced. In cases where new electrode arrays are required, the old arrays are removed, and new ones are inserted and connected to the IPG. The procedure also allows for the placement of additional electrode arrays at different sites if deemed necessary. Once all components are confirmed to be functioning correctly, the optimal settings for frequency and intensity of the impulses are programmed into the generator. Finally, the IPG is secured back into the pocket, and the incision is closed. In instances where the entire neurostimulation system is being removed, the pulse generator and electrode arrays are carefully extracted, and the incisions are sutured closed.

1. Indications

The procedure described by CPT® Code 0284T is indicated for patients experiencing issues with their permanent subcutaneous neurostimulator system. The specific indications for performing this procedure include:

  • Revision of Electrode Arrays - This is indicated when one or more electrode arrays are not effectively delivering electrical impulses to the area of maximal pain, necessitating repositioning or replacement.
  • Revision of Implantable Pulse Generator (IPG) - This is indicated when the IPG requires adjustments or repairs to ensure proper functionality and connection with the electrode wires.
  • Replacement of Electrode Arrays - This is indicated when it is determined that a new electrode array is necessary for effective pain management, requiring the removal of the old array and insertion of a new one.
  • System Removal - This is indicated when the entire neurostimulation system needs to be removed due to complications, device failure, or patient preference.

2. Procedure

The procedure for CPT® Code 0284T involves several detailed steps to ensure the effective revision or removal of the neurostimulator components. The steps are as follows:

  • Step 1: Incision Reopening - The existing incision over the subcutaneous pocket where the IPG is located is reopened to gain access to the device and electrode wires. This step is crucial for both revision and removal procedures.
  • Step 2: Exposure of the IPG and Wires - The implantable pulse generator and the distal ends of the electrode wires are carefully exposed. This allows for a thorough examination of the connections between the wires and the IPG.
  • Step 3: Checking Connections - The connections between the electrode wires and the IPG are checked for functionality. If any issues are identified, the connections may be repositioned or revised to ensure proper operation.
  • Step 4: Assessment of Electrode Arrays - The electrode arrays are assessed to determine if they are delivering impulses effectively. If they are not, the arrays are exposed for repositioning or revision.
  • Step 5: Replacement of Electrode Arrays - If it is determined that a new electrode array is needed, the old array is removed. The new array is then inserted, tunneled to the pocket, and connected to the IPG. Alternatively, a new electrode array may be placed at an additional site if required.
  • Step 6: Programming the IPG - Once all components are confirmed to be functioning properly, the optimal frequency and intensity of the electrical impulses are determined, and the generator is programmed accordingly.
  • Step 7: Closure of the Incision - The IPG is placed back into the pocket, secured to the underlying tissue, and the incision is closed with sutures. In cases where the entire system is being removed, the pulse generator is detached from the leads, and the electrode arrays are dissected from surrounding tissues before closing the incisions.

3. Post-Procedure

Post-procedure care following the revision or removal of the neurostimulator system involves monitoring the patient for any immediate complications, ensuring proper healing of the incision sites, and assessing the effectiveness of the revised or newly placed electrode arrays. Patients may be advised on activity restrictions to promote healing and prevent strain on the surgical sites. Follow-up appointments are typically scheduled to evaluate the functionality of the neurostimulator system and to make any necessary adjustments to the programming of the IPG. Additionally, patients should be educated on signs of infection or other complications that may require prompt medical attention.
