Official Description

Angiography, external carotid, bilateral, selective, radiological supervision and interpretation

Common Language Description

The procedure described by CPT® Code 75662 involves angiography of the external carotid arteries, which are the major blood vessels located on either side of the neck that supply blood to the head and face. In this procedure, a physician performs a selective angiography, meaning that they specifically target the external carotid arteries for imaging. To begin, a catheter is carefully inserted into both external carotid arteries. This is done to allow for the introduction of a contrast dye, which enhances the visibility of the blood vessels during imaging. Once the catheter is in place, the physician injects the dye into the arteries. As the dye circulates through the blood vessels, a series of X-ray images are captured. These images provide critical information about the structure and function of the external carotid arteries, helping to identify any abnormalities, blockages, or other vascular conditions. The procedure is performed under radiological supervision, ensuring that the images are interpreted accurately for diagnostic purposes.

1. Indications

Angiography of the external carotid arteries is typically indicated for a variety of clinical scenarios where detailed visualization of the blood vessels is necessary. The following conditions may warrant this procedure:

  • Suspected Vascular Abnormalities The procedure is performed when there is a suspicion of abnormalities such as stenosis, occlusion, or aneurysms in the external carotid arteries.
  • Evaluation of Head and Neck Symptoms Patients presenting with unexplained symptoms such as transient ischemic attacks (TIAs), strokes, or other neurological deficits may require this angiography to assess blood flow.
  • Preoperative Assessment Prior to surgical interventions in the head and neck region, angiography may be indicated to evaluate the vascular anatomy and plan for surgery.
  • Trauma Assessment In cases of trauma to the neck or head, this procedure can help identify any vascular injuries that may need to be addressed.

2. Procedure

The angiography procedure involves several key steps to ensure accurate imaging of the external carotid arteries. The following outlines the procedural steps:

  • Step 1: Patient Preparation The patient is positioned comfortably, and the area of catheter insertion is cleaned and sterilized to minimize the risk of infection. Sedation may be administered to help the patient relax during the procedure.
  • Step 2: Catheter Insertion A small incision is made, typically in the groin or wrist, to access a major artery. A catheter is then carefully threaded through the vascular system to reach the external carotid arteries on both sides of the neck.
  • Step 3: Contrast Dye Injection Once the catheter is in place, a contrast dye is injected through the catheter into the external carotid arteries. This dye enhances the visibility of the blood vessels during imaging.
  • Step 4: Imaging Acquisition As the contrast dye flows through the arteries, a series of X-ray images are taken. These images capture the anatomy and any potential abnormalities within the external carotid arteries.
  • Step 5: Catheter Removal and Recovery After the imaging is complete, the catheter is carefully removed. Pressure is applied to the insertion site to prevent bleeding, and the patient is monitored during the recovery period.

3. Post-Procedure

Following the angiography, patients are typically monitored for a short period to ensure there are no immediate complications, such as bleeding or allergic reactions to the contrast dye. Patients may experience some discomfort or bruising at the catheter insertion site, which usually resolves within a few days. It is important for patients to follow any specific post-procedure instructions provided by their healthcare provider, which may include restrictions on physical activity and hydration recommendations. Follow-up appointments may be scheduled to discuss the results of the angiography and any further management that may be necessary based on the findings.
