Official Description

Angiography, carotid, cerebral, bilateral, radiological supervision and interpretation

Common Language Description

The procedure described by CPT® Code 75671 refers to an angiography of the carotid arteries, which are the major blood vessels located on either side of the neck that supply blood to the brain. In this procedure, a physician performs a diagnostic imaging technique that involves the insertion of a catheter into both carotid arteries. The catheter is a thin, flexible tube that allows for the delivery of a contrast dye directly into the arteries. This dye enhances the visibility of the blood vessels during imaging. As the dye is injected, it travels through the arteries, allowing the physician to capture a series of X-ray images. These images are crucial for assessing the condition of the carotid arteries, identifying any blockages, narrowing, or other abnormalities that may affect blood flow to the brain. The procedure is performed under radiological supervision, ensuring that the images obtained are interpreted accurately for diagnostic purposes.

1. Indications

The angiography procedure described by CPT® Code 75671 is indicated for various clinical scenarios where assessment of the carotid arteries is necessary. The following conditions may warrant this procedure:

  • Suspected Carotid Artery Stenosis - This condition involves narrowing of the carotid arteries, which can lead to reduced blood flow to the brain and increase the risk of stroke.
  • Transient Ischemic Attack (TIA) - Patients who have experienced a TIA, often referred to as a "mini-stroke," may require this procedure to evaluate the carotid arteries for potential blockages.
  • Stroke - In cases of acute stroke, angiography can help determine the cause and extent of vascular occlusion or abnormalities.
  • Preoperative Assessment - Prior to certain surgical procedures, particularly those involving the heart or brain, angiography may be performed to assess the vascular status of the carotid arteries.

2. Procedure

The angiography procedure involves several critical steps to ensure accurate imaging and assessment of the carotid arteries. The following outlines the procedural steps:

  • Step 1: Patient Preparation - The patient is positioned comfortably, and the area where the catheter will be inserted is cleaned and sterilized to minimize the risk of infection. The physician may administer a sedative to help the patient relax during the procedure.
  • Step 2: Catheter Insertion - A local anesthetic is applied to the insertion site, typically in the groin or wrist. The physician then makes a small incision and inserts a catheter into the femoral or radial artery, guiding it through the vascular system to reach the carotid arteries.
  • Step 3: Contrast Dye Injection - Once the catheter is in place, a contrast dye is injected through the catheter into both carotid arteries. This dye is essential for enhancing the visibility of the arteries during imaging.
  • Step 4: Imaging Acquisition - As the contrast dye flows through the arteries, the physician captures a series of X-ray images. These images provide detailed views of the carotid arteries, allowing for the assessment of any abnormalities.
  • Step 5: Catheter Removal - After the imaging is complete, the catheter is carefully removed. Pressure is applied to the insertion site to prevent bleeding, and a bandage is placed over the area.

3. Post-Procedure

Following the angiography, patients are typically monitored for a short period to ensure there are no immediate complications, such as bleeding or allergic reactions to the contrast dye. Patients may experience some discomfort or bruising at the catheter insertion site, which usually resolves within a few days. It is important for patients to follow any specific post-procedure instructions provided by their healthcare provider, which may include restrictions on physical activity and hydration recommendations. Follow-up appointments may be scheduled to discuss the results of the angiography and any necessary further interventions based on the findings.
