Official Description

Intravascular ultrasound (non-coronary vessel), radiological supervision and interpretation; initial vessel

Common Language Description

Intravascular ultrasound (IVUS) is a diagnostic imaging technique used to visualize the interior of blood vessels. This procedure is particularly useful in assessing conditions such as constrictions or blockages within the vessels. During the IVUS procedure, a specialized ultrasound probe is carefully inserted into the blood vessel through a catheter. This allows for real-time imaging of the vessel walls and surrounding structures, providing critical information that can guide treatment decisions. The CPT® Code 75945 specifically refers to the radiological supervision and interpretation of the intravascular ultrasound for the initial vessel being examined. It is important to note that if additional vessels are treated during the same session, CPT® Code 75946 should be used to report each subsequent vessel. This coding structure ensures accurate billing and documentation of the procedures performed, reflecting the complexity and extent of the vascular assessment conducted.

1. Indications

The intravascular ultrasound (IVUS) procedure is indicated for various conditions related to blood vessels. These include:

  • Assessment of Vascular Blockages The procedure is performed to evaluate the presence and severity of blockages within blood vessels, which may impede blood flow.
  • Evaluation of Vascular Constrictions IVUS is utilized to examine areas of constriction in the vessels, helping to determine the appropriate treatment options.
  • Guidance for Interventional Procedures The imaging provided by IVUS aids in guiding interventional procedures, ensuring that treatments are accurately targeted to the affected areas.

2. Procedure

The intravascular ultrasound procedure involves several key steps that ensure accurate imaging and assessment of the blood vessels. These steps include:

  • Step 1: Catheter Insertion A catheter is carefully inserted into the blood vessel through a small incision. This catheter serves as the conduit for the ultrasound probe, allowing it to be positioned accurately within the vessel.
  • Step 2: Probe Deployment Once the catheter is in place, the ultrasound probe is advanced through the catheter and into the blood vessel. This probe emits high-frequency sound waves that penetrate the vessel walls, creating detailed images of the internal structures.
  • Step 3: Image Acquisition As the probe moves along the vessel, it captures real-time images of the vessel walls and any abnormalities present. These images are transmitted to a monitor for immediate review by the physician.
  • Step 4: Radiological Supervision and Interpretation A qualified radiologist supervises the procedure, ensuring that the imaging is performed correctly. After the images are obtained, the radiologist interprets the results, providing a comprehensive assessment of the vessel's condition.

3. Post-Procedure

After the intravascular ultrasound procedure, patients may be monitored for a short period to ensure there are no immediate complications. It is common for patients to experience some discomfort at the catheter insertion site, which typically resolves quickly. The physician will provide specific post-procedure care instructions, which may include recommendations for activity restrictions and signs of potential complications to watch for. Follow-up appointments may be scheduled to discuss the results of the ultrasound and any further treatment options that may be necessary based on the findings.
