Official Description

Angiography, carotid, cervical, bilateral, radiological supervision and interpretation

Common Language Description

The procedure described by CPT® Code 75680 involves angiography of the carotid arteries, which are major blood vessels located in the neck that supply blood to the brain, neck, and face. In this procedure, a physician performs a diagnostic imaging technique to visualize the internal structure of the carotid arteries. The process begins with the insertion of a catheter, a thin flexible tube, into both carotid arteries, which are situated on either side of the neck. Once the catheter is in place, a contrast dye is injected through it into the arteries. This dye enhances the visibility of the arteries on X-ray images. As the dye circulates through the blood vessels, the physician captures a series of X-ray images, allowing for detailed examination of the arteries' condition. This procedure is crucial for diagnosing various conditions affecting the carotid arteries, such as blockages, stenosis, or other vascular abnormalities, and is performed under radiological supervision to ensure accurate interpretation of the images obtained.

1. Indications

The angiography procedure described by CPT® Code 75680 is indicated for several specific clinical scenarios, particularly when there is a need to assess the condition of the carotid arteries. The following are the primary indications for performing this procedure:

  • Suspected Carotid Artery Disease - This includes symptoms such as transient ischemic attacks (TIAs) or strokes, where there is a concern for significant narrowing or blockage in the carotid arteries.
  • Evaluation of Carotid Stenosis - The procedure is used to measure the degree of narrowing in the carotid arteries, which can lead to reduced blood flow to the brain.
  • Preoperative Assessment - Angiography may be performed prior to surgical interventions, such as carotid endarterectomy, to provide detailed anatomical information about the arteries.
  • Monitoring of Known Carotid Conditions - For patients with previously diagnosed carotid artery disease, this procedure can help monitor the progression of the disease or the effectiveness of treatment.

2. Procedure

The angiography procedure as outlined in CPT® Code 75680 involves several critical steps to ensure accurate imaging of the carotid arteries. The following procedural steps are performed:

  • Step 1: Patient Preparation - The patient is positioned comfortably, and the area around the neck is cleaned and sterilized to minimize the risk of infection. An intravenous (IV) line may be established for sedation or medication administration as needed.
  • Step 2: Catheter Insertion - A local anesthetic is administered to numb the area where the catheter will be inserted. The physician then makes a small incision, typically in the groin or wrist, to access a major artery. A catheter is carefully threaded through the vascular system to reach both carotid arteries in the neck.
  • Step 3: Contrast Dye Injection - Once the catheter is positioned correctly in both carotid arteries, a contrast dye is injected through the catheter. This dye is essential for enhancing the visibility of the arteries during imaging.
  • Step 4: Imaging Acquisition - As the contrast dye flows through the carotid arteries, the physician takes a series of X-ray images. These images capture the anatomy and any potential abnormalities within the arteries, such as blockages or narrowing.
  • Step 5: Catheter Removal and Recovery - After the imaging is complete, the catheter is carefully removed, and pressure is applied to the insertion site to prevent bleeding. The patient is then monitored for a short period to ensure stability before being discharged.

3. Post-Procedure

Following the angiography procedure, patients are typically monitored for any immediate complications, such as bleeding or allergic reactions to the contrast dye. It is common for patients to experience some discomfort or bruising at the catheter insertion site. Instructions for post-procedure care may include recommendations to rest, avoid strenuous activities for a specified period, and stay hydrated to help flush the contrast dye from the body. Patients may also be advised to watch for any unusual symptoms, such as increased pain, swelling, or signs of infection at the insertion site, and to follow up with their healthcare provider for further evaluation and interpretation of the imaging results.
