Official Description

Laparoscopy, surgical; with placement of interstitial device(s) for radiation therapy guidance (eg, fiducial markers, dosimeter), intra-abdominal, intrapelvic, and/or retroperitoneum, including imaging guidance, if performed, single or multiple (List separately in addition to code for primary procedure)

Common Language Description

The procedure described by CPT® Code 49327 involves the laparoscopic surgical placement of interstitial devices, which are critical for guiding radiation therapy. These devices, including fiducial markers and dosimeters, are strategically positioned within the intra-abdominal, intrapelvic, or retroperitoneal areas to enhance the precision of radiation treatment. Fiducial markers, often made of gold, serve as reference points that allow radiation oncologists to accurately target malignant tumors while minimizing exposure to adjacent healthy tissues. This is essential in ensuring that the radiation is concentrated on the tumor, thereby maximizing treatment efficacy and reducing potential side effects. Implantable dosimeters, on the other hand, are utilized to measure the radiation dose delivered at the tumor site, providing valuable data for treatment planning and adjustment. The laparoscopic approach allows for minimally invasive access to the targeted areas, where imaging guidance is employed to confirm the correct placement of these devices. The procedure concludes with the removal of the laparoscope and trocars, followed by the closure of the incisions, ensuring a swift recovery for the patient.

1. Indications

The placement of interstitial devices for radiation therapy guidance is indicated in various clinical scenarios where precise targeting of malignant tissues is essential. The following conditions may warrant this procedure:

  • Malignant Tumors Patients with diagnosed malignant tumors in the abdomen, pelvis, or retroperitoneum may require fiducial markers to ensure accurate radiation delivery.
  • Radiation Therapy Planning Individuals preparing for radiation therapy may need dosimeters to measure radiation doses at the tumor site, aiding in treatment planning.
  • Need for Accurate Localization Cases where accurate localization of the tumor is critical for effective treatment may necessitate the use of interstitial devices.

2. Procedure

The procedure for the placement of interstitial devices involves several key steps, each crucial for ensuring the successful implantation of fiducial markers or dosimeters:

  • Step 1: Patient Preparation The patient is positioned appropriately on the operating table, and standard surgical preparations are performed, including anesthesia administration and sterilization of the surgical site.
  • Step 2: Laparoscopic Access Small incisions are made in the abdomen to allow for the insertion of trocars, which facilitate the introduction of the laparoscope and other surgical instruments.
  • Step 3: Imaging Guidance Imaging techniques, such as ultrasound or fluoroscopy, are utilized to identify the precise locations for the placement of the interstitial devices within the tumor or surrounding tissue.
  • Step 4: Device Placement One or more fiducial markers or dosimeters are carefully passed through the operating ports and positioned accurately in or around the tumor. The placement is confirmed through radiographic imaging to ensure correct positioning.
  • Step 5: Completion of Procedure After confirming the proper placement of the devices, the laparoscopic instruments are removed, and the incisions are closed with sutures, completing the procedure.

3. Post-Procedure

Following the laparoscopic placement of interstitial devices, patients are typically monitored for any immediate complications. Post-procedure care may include pain management and instructions for activity restrictions to promote healing. Patients are advised to follow up with their healthcare provider to assess the placement of the devices and to plan for subsequent radiation therapy. Recovery time may vary, but the minimally invasive nature of the procedure generally allows for a quicker return to normal activities compared to open surgical techniques.
